Personal Care at Homeland Center is tailored to provide our residents with the support they need to live an active and healthy life with as much independence as possible. The Personal Care team works hard to build a community of trust for every resident who calls Personal Care "home." At Homeland, every person on our team matters and fulfills a greater purpose in the lives of our residents, patients, and clients.

As a Med Tech for Homeland Center's Personal Care, you contribute to our culture of care and trust by providing medication administration assistance for residents within an independent living setting.

  • This is a part-time, Weekend opening, scheduled 6:00 pm - 6:00 am, Saturdays and Sundays.
  • Certification in Medication Administration Required, with a minimum of 2 years experience.

Assesses, plans and implements the nursing care of residents within the personal care nursing setting with the supervision of the Unit Manager. Responsible for ensuring continuity of care of the residents between shifts by providing direct care, as well as supervising the care given by CNAs and support staff members. Maintains role as resident advocate with a focus on the facility's mission, goals, and objectives.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned .

  • Assesses the resident's condition and nursing needs. Reports all information to the Unit Manager.
  • Documents changes in residents' condition and nursing care on progress notes and RASP. Reports concerns to the Unit Manager immediately.
  • Administers medications and follows the seven (7) medication rights to reduce the potential for medication errors.
  • Performs all aspects of resident care in an environment that optimizes resident safety and reduces the likelihood of medical/health care errors.
  • Supports and maintains a culture of safety and quality in all aspects of their work and requires subordinates to do the same.
  • Meets current documentation standards and policies, including but not limited to, use of proper charting techniques, accurately completing incident reports, 24-hour reports, etc.
  • Notifies physician of changes in resident's condition and follows through until appropriate action is taken.
  • Notes and carries out physician's orders in a timely manner.
  • Gives a thorough report to oncoming shift by verbal communication, 24-hour report documenting and documenting appropriately on calendar for upcoming needs.(i.e. labs, appointments, etc.)
  • Understands the importance of addressing resident skin related issues in a timely and appropriate fashion, i.e., completes appropriate charting/documentation. Notifies Unit Manager of any issues immediately.
  • Follows through as needed on information given by shift report, resident or family regarding resident concerns (i.e., contacting physician, family, performing assessment, etc).
  • Implements resident's RASP based on assessments and goals established therein.
  • Promptly responds to notifications relating to residents in distress or in pain
  • Treats residents and their families with dignity and respect. Identifies and addresses psycho-social, cultural, ethnic, and religious/spiritual needs of residents and their families.
  • Supervises and motivates other members of the healthcare team as needed to accomplish the resident's care, which has been developed and contained in their RASP.
  • Demonstrates competence in technical and manual skills according to scope of practice for Med Techs (i.e., Charting, Assessment, IMAR, etc.).
  • Functions as a resident advocate by protecting the resident's rights and dignity at all times and requires others to do the same.
  • Demonstrates creativity in solving problems and looking at situations in new and innovative ways.
  • Interacts with residents, families, co-workers, managers, subordinates and other departmental staff in a kind, considerate, knowledgeable, and friendly manner.
  • Consistently involves resident in their care and addresses their concerns while dispensing medications, treatments, and while performing assessments. Encourages staff to do the same.
  • Functions as a leader for the CNAs. Delegates, uplifts, reprimands/re-educates, acknowledges excellent care in a manner that contributes to the excellence and harmony of the unit. Consistently encourages and supports teamwork and cohesiveness whenever possible.
  • Participates in orientation of new staff and assists in performance evaluations, when needed.
  • Demonstrates ability to remain calm in emergency and/or stressful situations and gives everyone a feeling of confidence that the unit is under control.
  • Consistently follows the policies and procedures of the facility, as well as Personal Care Regulation.
  • Must have the ability and desire to acquire additional knowledge related to geriatric nursing, assessment techniques, etc. and apply the knowledge when necessary.
  • Is a motivated, independent individual with initiative who can organize workload so that all functions are completed appropriately, accurately, and timely.

PROFESSIONAL REQUIREMENTS:

  • Adheres to dress code, appearance is neat, clean and odor free.
  • Completes annual education requirements for Personal Care (12 hours) and competency assessments, as scheduled.
  • Maintains regulatory and certification requirements and standards at all times.
  • Maintains resident confidentiality at all times.
  • Reports to work on time and as scheduled; completes work within designated time; returns to unit in a timely fashion upon completion of lunch and breaks.
  • Wears identification while on duty, uses computerized punch time system correctly.
  • Attends annual review, departmental staff meetings & in-services, as scheduled or required, and returns to the unit in a timely fashion.
  • Adheres to all safety rules and regulations governing the facility and resident care.
  • Represents the organization in a positive and professional manner.
  • Ability to communicate effectively and professionally in English, both verbally and in writing.
  • Actively participates in an all QAPI related activities which may include working in a group or on a committee.
  • Complies with all organizational policies regarding ethical business practices and corporate compliance.
  • Communicates the mission, ethics, and goals of the facility with full understanding.

REGULATORY REQUIREMENTS:

  • Maintains certification in Medication Administration
  • Maintains certification in Diabetes Management
  • Maintains certification in CPR & First Aid

EDUCATION AND EXPERIENCE REQUIREMENTS:

  • A high school diploma or equivalent.
  • Experience in a healthcare environment, passing medications preferred.
  • Successful completion of DPW Direct Care Training Program.
  • Successful completion of the DPW Medication Administration Course.
  • The Med Tech will maintain current competency associated with the DPW Medication Administration Course.
  • The emotional stability to deal with confused, combative, disoriented residents calmly and professionally.
  • Must be at least 21 years of age

Language Skills:

  • Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to effectively present information in one-on-one and small group situations to customers, clients, and other employees of the organization.

Mathematical Skills:

  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

Reasoning Ability:

  • Ability to apply common sense understanding to carry out detailed but uninvolved written or oral instructions. Ability to deal with problems involving a few concrete variables in standardized situations.

Computer Skills:

  • Experience and or familiarity with computerized documentation is a plus.

Certificates, Licenses, Registration:

  • Graduation Certificate from a Pa state approved Nurse Aide Training Program.
  • Current Nurse Aide Certificate from the Pa Nurse Aide Registry.

Other Skills and Abilities:

  • Compassion and Caring
  • Skill with geriatric patients or patients with disabilities
  • General understanding of medical terminology
