We are seeking a Full-Time Nurse Practitioner (FNP, AGNP, PMHNP, or equivalent) to join our expanding team in Albuquerque, NM. The ideal candidate is professional, compassionate, and clinically confident, with strong communication skills and a commitment to patient-centered care. Experience in primary care, geriatrics, mental health, or chronic disease management is highly valued. In this role, you will utilize your advanced clinical skills to assess, diagnose, and manage patient health needs, ensuring comprehensive care tailored to each individual. Your expertise will support a wide range of services, from acute care to chronic disease management, in collaboration with physicians and interdisciplinary teams. Duties
- Conduct comprehensive assessments, diagnose conditions, and develop treatment plans
- Provide primary and urgent care services depending on specialty
- Manage chronic and acute medical conditions
- Prescribe medications and order diagnostic tests
- Collaborate with interdisciplinary team members to ensure coordinated patient care
- Maintain accurate and timely documentation in EHR
- Participate in quality improvement initiatives and team meetings
- Provide patient education, health promotion, and disease prevention counseling
- Utilize EMR (Electronic Medical Records) systems to document patient encounters accurately while maintaining HIPAA compliance.
- Provide telehealth consultations to expand access to care and coordinate follow-up through case management and discharge planning.
- Assist with diagnostic evaluations including laboratory specimen collection/processing, sonography, medical imaging interpretation, and basic life support procedures.
- Collaborate with multidisciplinary teams on complex cases involving ICU or Level I Trauma Center experience, hospital medicine, or post-acute care settings.
Requirements
- Valid Nurse Practitioner license with current state certification; additional certifications in family practice or specialty areas preferred.
- Valid DEA license
- Ability to work independently and collaboratively
- Proven experience in medical office settings with familiarity in EHR systems.
- Strong clinical skills including patient assessment, medication administration, IV insertion, phlebotomy, and sterile technique.
- Knowledge of ICD-10/ICD-9 coding and CPT coding for billing purposes; experience with Workers' Compensation law is a plus.
- Experience working with diverse populations such as assisted living residents, nursing home patients, hospice & palliative care clients, or individuals with developmental disabilities.
- Familiarity with infection control protocols and infection prevention practices across inpatient and outpatient settings.
- Ability to work effectively within multidisciplinary teams providing geriatrics, pediatric (including neonatal ventilator), behavioral health or memory care services.
