Case Manager

Full Time Position

Monday-Friday

8:00am-5:00pm

The Palliative Care Case Manager (PCCM) is an important member of the Palliative Care inpatient consulting service and is responsible for providing first responder assessment and support to new patient referrals, acting as an advocate for the patient and family through collaboration with the members of the treatment team. The PCCM provides emotional support, psychosocial assessment, education and facilitation of advanced care planning as well as contributes to development of discharge planning, care coordination, and follow-up. Identification of patient and family needs and services post hospitalization are discussed with the palliative patients to include hospice care when appropriate.

Required Education: Bachelor's degree Specific course work: Candidates must have a Bachelor's degree in Nursing, OR Masters in Social Work or related field.

Required Licensure/Certifications: Licensed to practice nursing in the state of Florida for those meeting the BSN education requirement Masters in Social Work (or related field) accepted in lieu of RN licensure

Preferred Licensure/Certifications: Palliative Care Specialty certification OR License Clinical Social Work

Required Experience: 1-2 years of experience in Palliative or hospice care or complex case management experience

Preferred Experience: 2 + years working with hospice and/or palliative care patients

Necessary Skills:

-Excellent verbal and written communication skills

-Excellent organizational skills

-Strong personal computer skills (i.e., Email, Word, Excel)

-Good typing skills
