Responsibilities as a Licensed Social Worker (LSW)

• Evaluate patients (existing, new, and prospective) based on their needs and limitations.
• Address patient concerns and goals while maintaining constant communication with the patient, as well as the case manager.
• Build rapport with patients and their families.
• Develop care plans for patients.

Qualifications of a Licensed Social Worker (LSW)

Must have a CSW or LSW

• College degree
• Must have long-term care experience
