LVN Intake Coordinator

The LVN Intake Coordinator plays a critical role in the hospice care team by managing the initial patient intake process to ensure seamless admission and high-quality care delivery. This position requires thorough assessment of patient needs, coordination with clinical staff, and effective communication with patients and their families to facilitate a compassionate and efficient transition into hospice services. The coordinator will serve as a liaison between referral sources, patients, and interdisciplinary teams to gather and verify necessary documentation and clinical information. By maintaining accurate records and coordinating timely admissions, the LVN Intake Coordinator helps optimize patient outcomes and supports compliance with regulatory standards. Ultimately, this role ensures that patients receive personalized, respectful, and timely hospice care from the very beginning of their journey.

As a Intake Referral Coordinator, you will:

• Receives and appropriately documents new referrals in a timely manner.
• Verifies each patient's payor source and maintains and tracks insurance authorizations and initial certifications.
• Participates in coordinating care with Nursing Supervisors, patients, and the interdisciplinary group.
• Provides effective communication to patients, staff members, other health care professionals and referral sources.
• Responds to Agency and patient needs in a professional and creative manner.
• Demonstrates commitment, professional growth and competency.
• Participates in Agency-sponsored in-service training.
• Participates in the Agency's QAPI program.
• Promotes the Agency philosophy and administrative policies to ensure quality of care.

Requirements

• Minimum one year of experience in referral intake is preferred
• Experience with third-party payers, Medicare and Medicaid highly desirable
• Medical Field background preferred, including familiarity with medical terminology
• Excellent computer and customer service skills
• LVN license in Texas
• Valid Driver's License
• Valid Auto Insurance
• HomeCare HomeBase experience preferred
