Description of Work
The Early Intervention Service Coordinator (EISC) provides professional-level service coordination for infants and toddlers, birth to age three, who are enrolled in the North Carolina Infant-Toddler Program and have or are at risk for developmental delays or disabilities. Working within a Children's Developmental Services Agency (CDSA), the EISC facilitates a family-centered approach to assessment, planning, and service delivery in compliance with federal and state regulations. The position involves developing and monitoring Individualized Family Service Plans (IFSPs), coordinating multidisciplinary services, and supporting families in accessing resources and navigating transitions. EISCs work collaboratively with families, service providers, and community agencies to promote optimal child development. Frequent local travel is required to deliver services in natural environments such as homes and childcare settings.Key Duties Include:
-Conduct initial contacts with families to explain program procedures, obtain consents, and gather intake and financial information.
-Coordinate and participate in eligibility determination by scheduling evaluations, collecting documentation, and facilitating family-directed assessments.
-Develop comprehensive IFSPs in collaboration with families and multidisciplinary teams, identifying outcomes, services, and supports.
-Authorize and coordinate early intervention services, including arranging provider access and ensuring timely service delivery.
-Monitor IFSP implementation through regular reviews, provider communication, and documentation of child progress and service quality.
-Facilitate ongoing assessments and evaluations to determine continued eligibility and adjust services as needed.
-Support families in identifying and accessing community resources, including medical, developmental, and social supports.
-Educate and empower families to advocate for their child's needs and understand developmental milestones and intervention strategies.
-Coordinate transition planning as children approach age three, ensuring compliance with federal guidelines and facilitating referrals to other programs.
-Maintain accurate and timely documentation in accordance with program standards and participate in quality assurance activities.
Knowledge Skills and Abilities/Management Preferences
Salary Grade Range: $35,984.00 – $62,971.00Candidates now meet the minimum qualifications of a position if they meet the minimum education and experience listed on the vacancy announcement.
The Knowledge, Skills, and Abilities (KSAs)/ Management Preferences are not required. Applicants who possess the following skills are preferred:
Documented experience conducting multidisciplinary developmental evaluations for children from birth to age three, including coordinating team participation, integrating assessment results, and ensuring compliance with evaluation protocols.
Proven experience developing and implementing Individualized Family Service Plans (IFSPs) in collaboration with families and service providers, with consistent adherence to federally mandated timelines and measurable child and family outcomes.
Hands-on experience using electronic case management systems to track service coordination activities, monitor provider documentation, and maintain compliance with procedural safeguards and reporting standards.
Experience facilitating transitions from early intervention services to preschool or community-based programs, including coordinating referrals, preparing families, and ensuring timely completion of required documentation.
Demonstrated experience coaching families and caregivers using evidence-based practices such as natural learning environment strategies and embedded interventions, resulting in increased caregiver engagement and developmental progress.
Must be able to drive and travel by car across the CDSA's catchment area.
