Service Area: West Michigan – including Grand Rapids and nearby communities.

We're seeking a Podiatry-focused Nurse Practitioner, Physician Assistant, or Podiatrist (DPM) to support patients in the geriatric population. This role centers on routine podiatry and foot-care management—no surgical procedures required.

The provider will deliver in-home care, help monitor ongoing conditions, and promote overall health and safety for older adults. Our need is specifically for coverage across Grand Rapids and the western Michigan region.

You will play a key part in patient wellness by performing assessments, reviewing labs, and monitoring foot and skin integrity for those who may be at high risk. Alongside hands-on care, you'll also guide patients and families toward healthier practices and preventative habits.

Primary Responsibilities

  • Conduct patient evaluations and review medical histories
  • Order, interpret, and follow up on diagnostic tests
  • Identify podiatric and wound-related conditions and develop care plans
  • Prescribe appropriate medications and therapies
  • Maintain accurate documentation and scheduling
  • Educate patients and caregivers regarding conditions, medications, and preventative care
  • Encourage safe and healthy habits within the home setting

Qualifications

  • Experience working as an NP, PA, or DPM in primary, acute, or chronic care settings
  • Familiarity with podiatry-focused care and geriatric needs
  • Strong understanding of patient-care guidelines and compliance requirements
  • Excellent interpersonal and communication skills
  • Professional integrity, reliability, and the ability to work independently
  • Willingness to continue learning and strengthening clinical skills
  • Current state licensure (NP, PA, or DPM)
  • Master's degree required for NP/PA applicants

Job Types: Full-time, Part-time

Pay: $95, $190,000.00 per year

Expected hours: 40 per week
