Job Overview
We are seeking a dedicated and skilled Physical Therapist to join our healthcare team. The ideal candidate will provide comprehensive patient care across various settings, including inpatient, outpatient, hospital, home health, and community care. This role involves developing and implementing individualized care plans to improve mobility, reduce pain, and enhance overall quality of life for diverse patient populations such as pediatrics, geriatrics, orthopedics, and those recovering from acute care episodes. The Physical Therapist will utilize their expertise in physiology, anatomy, and medical terminology to deliver effective physiotherapy interventions and discharge planning.

Duties

• Assess patients' physical conditions through detailed evaluations of mobility, strength, and functional abilities
• Develop personalized care plans based on patient needs and treatment goals
• Implement evidence-based physiotherapy techniques including trigger point therapy, orthopedics, geriatrics, and pediatric care
• Monitor patient progress and adjust treatment strategies accordingly
• Educate patients and their families on exercises, injury prevention, and recovery processes
• Coordinate with healthcare teams to facilitate discharge planning and transition of care from hospital to home or community settings
• Document patient interactions accurately using EMR systems
• Provide home & community care services as needed in various environments
• Collaborate with physicians and other healthcare professionals to optimize patient outcomes
• Maintain compliance with healthcare regulations and standards

Requirements

• Valid state licensure as a Physical Therapist with current registration
• Proven experience in acute care, inpatient, hospital, home health, or outpatient settings
• Strong knowledge of physiology, anatomy, and medical terminology
• Experience with care plans, discharge planning, and treatment for diverse populations including pediatrics and geriatrics
• Familiarity with various physiotherapy techniques such as trigger point therapy and orthopedic rehabilitation
• Proficiency in using EMR systems for documentation and patient management
• Excellent communication skills for patient education and interdisciplinary collaboration
• Ability to work independently while maintaining a team-oriented approach
