Job Title: Visiting Nurse Practitioner
As a Visiting Nurse Practitioner, you will play a vital role in delivering high-quality healthcare to patients in their homes and various healthcare settings. You will be the driving force behind compassionate patient care, utilizing your clinical expertise to assess, diagnose, and treat a diverse range of health conditions. Your proactive approach will ensure that patients receive the best possible care tailored to their unique needs, enhancing their overall well-being.
What you'll do
- Conduct comprehensive patient assessments and physical examinations to determine health status.
- Develop and implement individualized care plans based on patient needs and preferences.
- Administer treatments including IV infusion, medication administration, and botulinum toxin treatments as necessary.
- Collaborate with multidisciplinary teams to coordinate care across various settings such as assisted living facilities, nursing homes, and outpatient clinics.
- Provide education and support to patients and families regarding health management, medication adherence, and lifestyle modifications.
- Perform diagnostic evaluations including suturing, phlebotomy, and specimen collection/processing.
- Manage acute pain effectively while ensuring adherence to safety protocols in all procedures.
- Maintain accurate medical documentation in compliance with HIPAA regulations and organizational standards.
Basic qualifications
- Current Nurse Practitioner licensure with relevant certifications.
- Strong knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation techniques.
Preferred qualifications
- Experience with electronic medical record (EMR) systems such as Epic or Cerner.
- Background in critical care or emergency medicine with exposure to Level I or II trauma centers.
- Familiarity with CPT coding, ICD-9/ICD-10 coding systems, and workers' compensation law.
- Proficiency in telehealth practices and remote patient monitoring technologies.
- Experience in geriatrics or pediatrics, particularly with developmental disabilities or memory care.
