Job Overview
Cater To You Home Care is seeking a highly organized and compassionate Client Care Coordinator to support our clients, caregivers, and internal team. This position plays a key role in ensuring quality care delivery, timely communication, accurate documentation, and a professional client experience. The ideal candidate will be detail-oriented, proactive, and comfortable working in a fast-paced environment.

Responsibilities Client Intake & Case Coordination

  • Conduct new client intake calls and complete intake documentation
  • Verify eligibility for Medicaid, private pay, or waiver services
  • Schedule in-home assessments with RN or Administrator
  • Maintain accurate client records in agency systems
  • Manage client communication regarding services, scheduling, and updates
  • Follow up on client concerns and document outcomes

Caregiver Support

  • Assist with caregiver-client matching based on skills and availability
  • Support caregivers with questions, care plan clarification, and scheduling needs
  • Document and report caregiver issues or performance concerns

Scheduling & Service Coordination

  • Assist scheduler with daily coverage, call-offs, and shift changes
  • Communicate schedule updates to clients and families
  • Ensure care is delivered according to authorized hours and care plans

Documentation & Compliance

  • Record all communication in client and caregiver files
  • Log complaints, incidents, and follow-up actions
  • Prepare documentation for audits and quality assurance reviews
  • Maintain compliance with state, Medicaid, and agency requirements

Client Relations & Retention

  • Conduct routine client satisfaction check-ins
  • Identify and escalate changes in condition to RN/Administrator
  • Provide information on available resources or programs
  • Uphold high standards for customer service and professionalism

Qualifications

  • Prior experience in home care, healthcare coordination, or senior services preferred
  • Strong communication and interpersonal skills
  • Excellent organization and multitasking abilities
  • Competence using computers, scheduling systems, and EVV platforms
  • Ability to remain calm and professional under pressure
  • High attention to detail and strong follow-through
  • Must pass background check

Work Schedule

  • Monday–Friday
  • 8:30 AM–5:30 PM
  • 1-hour lunch break (12:30 PM–1:30 PM)
