Position Summary
ChesPenn Health Services is seeking a patient-focused Licensed Practical Nurse (LPN) to support clinical care in a Federally Qualified Health Center (FQHC) setting. This position is based at the Center for Family Health in Chester, PA, a busy primary care practice providing adult medicine, pediatrics, and women's health services. The LPN works as part of a multidisciplinary care team supporting a diverse and underserved patient population.
Qualifications
- Completion of an accredited Practical Nursing Program
- Current, unrestricted Pennsylvania Licensed Practical Nurse license
- Current Basic Life Support (BLS) or CPR certification
- Prior experience as an LPN in an outpatient or community health setting preferred
- Experience working with diverse, underserved, or high-risk populations preferred
- Current COVID-19 and influenza vaccination required
Knowledge, Skills, and Experience
- Ability to perform a variety of clinical support tasks in a fast-paced primary care environment
- Demonstrated ability to obtain and document patient histories and vital signs
- Demonstrated ability to perform venipuncture and CLIA-waived laboratory testing
- Demonstrated ability to administer injections and vaccinations per protocol
- Knowledge of infection control, safety, and sanitation standards
- Understanding of HIPAA confidentiality requirements and patient privacy best practices
- Familiarity with electronic health records and clinical documentation
- Excellent communication and interpersonal skills
- Ability to work collaboratively within a multidisciplinary care team
- Compassionate, patient-centered approach with the ability to manage stress effectively
What You'll Do
- Perform clinical duties under the direction of a licensed provider or Registered Nurse
- Prepare patients for provider visits by obtaining medical history, vital signs, growth parameters, and reason for visit
- Assist providers during examinations and procedures
- Administer injections and vaccinations according to clinical protocols
- Perform venipuncture and collect specimens for laboratory testing
- Conduct visual and auditory screenings as appropriate
- Accurately document patient care and clinical activities in the electronic health record
- Support quality improvement initiatives and compliance with HRSA, state, and federal requirements

- Demonstrate teamwork and professionalism in support of co-workers and organizational goals
- Occasional evening or weekend hours may be required based on operational needs
