Job Overview
We are seeking a dedicated and skilled Nurse Practitioner to join our healthcare team. The ideal candidate will have experience in various clinical settings, including nursing homes and ICU, and possess a strong foundation in primary care. This role requires the ability to perform diagnostic evaluations, manage patient cases, and provide comprehensive care to patients with diverse medical needs.
Duties
- Conduct thorough patient assessments, including taking vital signs and performing diagnostic evaluations.
- Develop and implement individualized care plans based on patient needs and medical history.
- Provide primary care services, including routine check-ups, health screenings, and preventive care.
- Manage complex cases through effective case management strategies.
- Perform catheter care and other necessary procedures as required.
- Collaborate with healthcare teams to ensure optimal patient outcomes.
- Participate in utilization review processes to assess the appropriateness of care provided.
- Educate patients and families on health management and disease prevention strategies.
- Maintain accurate patient records and documentation in compliance with healthcare regulations.
Requirements
- Current Nurse Practitioner license in the state of practice.
- Experience in nursing home settings or ICU preferred.
- Proficient in performing vital signs assessments and diagnostic evaluations.
- Strong skills in case management and utilization review processes.
- Ability to ventilate patients when necessary and perform catheter care effectively.
- Excellent communication skills for patient interaction and collaboration with healthcare teams.
- Must be able to lift patients as needed during assessments or procedures.
