We are a 32 unit Memory Care community looking for a strong LPN to join our team.
- The Licensed Practical Nurse (LPN) practices according to the philosophy, policies, and procedures of the community under the direction of the community Executive Director or DON. The Staff LPN assumes the responsibility of charge nurse as assigned, supervises and assigns/directs staff according to their job descriptions.
- The staff LPN assists the DON with nursing responsibilities and tasks, and administers medications.
- Job duties may included, but not limited to:
- Orientation, and supervision of Resident Care Attendants.
- Assists in the implementation of and/or providing additional information for the
- resident service plans and updating plans as needed on an ongoing basis.
- Assists Resident Care Attendants with the implementation of service plans. Provides assignment sheets to the RCAs at the start of shift to ensure completion of tasks.
- Oversees the provision of assistance with activities of daily living to residents whose service plan indicates the need.
- Communicates pertinent resident information to co-workers when leaving the unit and/or when the shift changes.
- Assumes responsibility for quality resident care and seeks guidance from supervisory personnel when necessary.
- Communicates with physicians regarding a resident's health and personal care needs when necessary and appropriate.
- Assists in scheduling lab tests or diagnostic tests ordered by physicians.
- Responds to emergency calls from residents immediately when on duty.
- Welcomes new residents and orients them to the community and resident care services available at the facility when on duty.
- Assists in evaluating residents' appropriateness for the facility regarding residents' safety and comfort in an assisted living residence and communicates with resident, family and supervisor as appropriate.
- Addresses family concerns, mediates or intervenes as appropriate, and communicates to supervisor(s).
- Sits, bends, life and moves intermittently during working hours.
- Works beyond normal hours and may be requested to rotate on weekends and holidays as needed and/or requested.
- Position Requirements Must be a Licensed Practical Nurse in the State of Illinois, and must demonstrate competency in the following areas:
- Supervising staff with leadership experience.
- Assessing health care needs.
- Assessing resident care needs.
- Establishing and implementing plans to meet needs.
- Communication skills, both written and oral.
- Interpersonal relationships.
- Attends continuing education courses as required.
- Scheduling of staff and supervision of staff.
Must be Vaccinated.
Job Types: Full-time, Part-time
Pay: $ $29.00 per hour
