This is a Hybrid position - you work from your home but travel to see clients.
Join our team and make a difference Our case managers serve individuals with intellectual and developmental disabilities to ensure their needs, desires, and goals are honored through a person-centered approach. The case manager works with individuals, families, service providers, and other stakeholders to assess needs, provide information, referrals and support, identify opportunities and facilitate the individual's participation in the community of his/her choice. The case manager coordinates with SC Department of Disabilities and Special Needs, Medicaid, school systems, service provider networks, and other systems to conduct intake duties, authorize services, implement and monitor plans, and respond to an individual's needs on ongoing basis. Case managers incorporate a spirit of advocacy in all that they do to promote self-determination. Organization, refined communication skills, task completion and time management skills are vital to this role. Travel is required in order to monitor individuals on the caseload throughout the service area. Florence area is desired.

Minimum Qualifications:
*Bachelor's degree in social work or other related field from an accredited college or university and documentation of at least one year of experience working with people with disabilities and/or case management are required, as per state regulations.*
Essential Job Duties:
- Ability to understand and adapt to changing laws and policies, and clearly communicate these policies to clients and their families.
- Ability to clearly communicate verbally, both in personal and over the telephone.
- Ability to use a computer to accurately and expediently document case notes, navigate databases, send and receive email, and conduct research regarding resources, information, and other relevant material.
- Valid Drivers License and Ability to travel in surrounding areas (mileage is compensated).
Knowledge & Skills:
- Demonstrated ability to advocate and problem-solve with clients, while implementing a person-centered approach which fosters self-determination.
- Demonstrated organizational and time management skills.
- Provide holistic and comprehensive case management services to all clients.
- Excellent communication skills; verbal and written, technical and non-technical.
- Flexibility in adapting to ever-changing laws, policies and procedures within the service delivery system.
- Ability to work with and collaborate with co-workers and others.
- Motivation to make change in peoples lives
Job Type: Full-time
