Case Manager

Job Summary:

Responsibilities:
- Conduct patient assessments to determine their needs and develop individualized care plans
- Collaborate with healthcare professionals, including physicians, nurses, and social workers, to coordinate patient care
- Monitor patient progress and adjust care plans as needed
- Provide education and support to patients and their families regarding treatment options and resources
- Advocate for patients' rights and ensure they receive appropriate care
- Maintain accurate and up-to-date documentation of patient interactions and progress
- Stay current on relevant medical research, treatment options, and community resources

Requirements:
- Bachelor's degree in a related field (e.g., Social Work, Nursing)
- Previous experience in case management or a related role preferred
- Knowledge of ICU protocols and acute care settings
- Familiarity with physiology and medical terminology
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Knowledge of addiction counseling principles is a plus
- Understanding of HIPAA regulations and patient confidentiality
- Proficiency in using electronic medical records systems
