*Job Summary*
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will provide high-quality patient care across various settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. Your expertise will support patient recovery, comfort, and well-being through skilled nursing interventions, assessments, and personalized care plans. *Responsibilities*
* Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care plans.
* Administer medications and injections accurately following prescribed protocols while ensuring patient safety and comfort.
* Assist with medical procedures such as catheterization, tube feeding, venipuncture, and sterile dressing changes using aseptic techniques.
* Monitor patients’ conditions closely, utilizing knowledge of physiology and diagnostic evaluation to identify changes requiring intervention.
* Maintain detailed medical documentation within electronic health record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA regulations.
* Support complex patient needs including acute pain management, dementia care (including Alzheimer’s and memory care), hospice support, and infectious disease care.
* Collaborate with physicians, case managers, and interdisciplinary teams to coordinate discharge planning and post-acute care strategies.
* Provide education on health coaching topics such as medication management, nutrition, mobility assistance (Hoyer lift), and chronic disease management.
* Assist in emergency situations by managing airway (airway management), ventilator support, or basic life support (BLS) procedures as needed.
* Engage with diverse patient populations including pediatrics, geriatrics, individuals with developmental disabilities or disabilities requiring specialized care such as spinal tap or neonatal ventilator support.
*Experience*
* Valid state licensure as a Licensed Practical Nurse (LPN) with current certification.
* Proven experience in various healthcare environments such as hospitals (including ICU or Level I Trauma Centers), nursing homes, long-term care facilities, or outpatient clinics.
* Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks for accurate documentation and patient records management.
* Hands-on experience administering injections, phlebotomy (venipuncture), catheter care, and specimen collection/processing.
* Knowledge of ICD coding (ICD-9/ICD-10), CPT coding for procedures and treatments is preferred for documentation accuracy.
* Familiarity with specialized areas such as emergency medicine, critical care (ICU), trauma response (Level II trauma center experience), geriatrics or behavioral health is highly valued.
* Strong understanding of medical terminology, anatomy knowledge, physiology principles and diagnostic evaluation techniques.
* Experience working with elderly populations , neonatal units or individuals with complex conditions like spinal tap procedures or gastrostomy tube feeding. Pay: $30.00 - $32.00 per hour
