Job Overview
We are seeking a compassionate and dedicated Case Manager - QMHP to join our mental health and behavioral health services team. The ideal candidate will provide comprehensive case management, coordinate care, and support individuals with diverse mental health needs. This role involves working closely with clients across various settings, including outpatient clinics, inpatient facilities, and telehealth platforms. The successful applicant will utilize their expertise in clinical counseling, crisis intervention, and behavioral management to promote recovery and well-being for individuals of all ages, including children, adolescents, adults, and geriatrics. Experience with medical documentation and HIPAA compliance is essential to ensure accurate record-keeping and effective treatment planning.

Responsibilities

  • Develop individualized care plans incorporating evidence-based practices such as cognitive behavioral therapy, psychodynamic therapy, art therapy, dance therapy, and applied behavior analysis.
  • Provide individual and group counseling sessions addressing issues such as grief, trauma (including PTSD), addiction, anxiety disorder, depression, behavioral concerns.
  • Implement crisis management strategies and crisis intervention techniques to support clients experiencing acute mental health crises.
  • Coordinate services across multidisciplinary teams to ensure continuity of care in outpatient, inpatient, telehealth settings or community-based programs.
  • Engage in research activities related to behavioral health outcomes or clinical research projects to inform best practices.
  • Support clients through intake processes and facilitate referrals for specialized services such as social work support or psychiatric medication management.
  • Collaborate with families and caregivers to provide child & family counseling tailored to developmental needs and family dynamics.
  • Utilize skills in motivational interviewing, crisis management, behavior management, and social work to foster client engagement and progress toward recovery goals.

Requirements

  • Valid Qualified Mental Health Professional (QMHP) certification or equivalent licensure required.
  • CANS and ANSA certification
  • CPR Certification
  • Proven experience in clinical counseling modalities including psychotherapy, cognitive behavioral therapy, psychodynamic therapy, addiction counseling, grief counseling, or art/dance therapy preferred.
  • Demonstrated expertise working with diverse populations such as children & families, individuals with developmental disabilities or autism spectrum disorder, geriatrics, or those experiencing trauma or PTSD.
  • Familiarity with telehealth platforms and remote service delivery models is highly desirable.
  • Excellent crisis intervention skills along with experience managing complex behavioral health cases across inpatient or outpatient settings.
  • Strong communication skills combined with a compassionate approach to patient care; experience working within multidisciplinary teams is a plus. Job Type: Part-time

Pay: $ $26.00 per hour

Expected hours: 4 – 15 per week
