Job Responsibilities
Assists the RN in carrying out the plan of care for each patient assigned
Provides patient care assigned and under the direction of the registered nurse who provides on-site supervision as needed, based upon severity of patients, medical condition and training, and the nurse's experience, according to Bay Nursing Inc. policies and procedures
Assists in the development and implementation of the interdisciplinary patient care plan as it pertains to nursing
Administers medication as defined in the medication policy, and treatments, as prescribed by the physician
Updates medication profile, and home chart on an ongoing basis
Prepares records and submits clinical and progress notes in a timely manner following visit, or communication
Maintains confidentiality of OASIS data set, patient medical records, all patient protected health information, and Bay Nursing Inc. material and business
Provides skilled nursing care as outlined in the patient plan of care to include the following:
- Nursing services, treatments and preventative procedures requiring substantial, specialized skill as directed by the RN
- Observing signs and symptoms and reporting to the RN reactions to treatments, including drugs, and changes in the patient's physical or emotional status
- Teaching, supervising, and counseling the patient and family members regarding the nursing care needs and other related problems of the patient at home under the RN's direction
- Assists patients in learning self-care techniques
- Participates in multidisciplinary approach to patient care
- Assists the MD and RN in performing specialized procedures
Communicates with the physician, documents appropriate change orders and/or communication. Informs the RN, and other inter-disciplinary team members of communications and/or changes, as indicated
Prepares equipment and materials for treatments, observing aseptic technique as required
Does not increase or decrease visit frequency or discharge patients without the RN's approval
Provides to the RN, a report regarding the patient's current physical and emotional status, any change in the patient's condition, visit schedule, required care and any noted need to change to the Plan of Care, at a minimum of weekly, and as deemed necessary by a change in the patient's condition
Demonstrates ability to communicate and receive constructive feedback in a positive manner
Ensures safe delivery of quality patient care in the home setting in a cost-effective manner
Participates in case conferences/coordination, care communication, in-service meetings, and other training sessions as directed by the Clinical Director
Participates in Performance Improvement activities and identifies areas for improvement
Demonstrates working knowledge of biohazardous waste disposal procedures, specimen transport and disposal procedures
Complies with infection control measures including appropriate bag technique, and hand decontamination process per company policy
Demonstrates proper use of required safety equipment, and proper body mechanics (e.g., use of seat belts and personnel protective equipment)
Demonstrates proper aseptic technique as appropriate care
Complies with Departmental and Company Policies and Procedures
Refrains from giving patient/family cell or home phone numbers, and discussing personal and/or company business with the patient
Adheres to the Agency policy with regards to the dress code
Maintain standards of practice and documentation that are consistent with Federal, State, Local, Medicare, and accreditation regulations and standards
Demonstrate flexibility while performing job duties to ensure the highest quality of service is provided
Utilize critical thinking skills while performing job duties; analyze work process and make suggestions for improvement
Maintain a thorough understanding and consistently adhere to all applicable department and company policies and procedures
Conduct job duties in a way which will reduce and avoid potential safety hazards
Demonstrate initiative and drive to promote personal and company success
Perform other duties as assigned by your supervisor or management team member
Education and Experience Required
Graduate of an accredited practical nurse or vocational nursing program
Has two years nursing experience. Home health experience is required
Currently licensed as a licensed practical nurse or licensed vocational nurse in the state
Complies with accepted professional standards and practice
Possesses and maintains current CPR certification
