Description

Position summary:

Under general supervision, the Care Coordinator operates as part of the Care Team, working collaboratively with providers and clinical staff, internal and external, to support patients with chronic conditions and complex needs. They may serve as a liaison for needed social services, and are responsible for coordinating referrals, consults and diagnostic testing as well as tracking results of these appointments. Under general supervision they may provide assistance with prescription medication assistance applications as well as other benefit. They are also responsible for communicating with patients that are considered overdue for care.

Job Responsibilities:

  • Demonstrates and applies knowledge of the philosophy/principles of culturally sensitive and comprehensive coordination of care.
  • Maintains active contacts with community partners to ensure easy facilitation of referrals
  • Assists in facilitating and processing prescription assistance requests.
  • Assists in scheduling referrals, consults and diagnostic testing.
  • Follows up and tracks the outcome of referrals, consults, and diagnostic testing.
  • Effectively communicates with health care team within the clinic and the community to provide quality services.
  • Contacts clients overdue for care and encourages appropriate follow up care.
  • Maintains regular contact with other social service agencies and health care providers involved with client care to provide information and obtain feedback on client's overall progress.
  • Documents all contacts and follow ups with clients and other providers daily.
  • Performs miscellaneous duties as assigned.
Qualifications

Minimum Qualifications:  

High school graduate with at least 3 years of experience in a generalized public health care setting.

Preferred Qualifications:

  • Associates degree with 1 year of health care experience preferred.
  • Skills in using word processing software for composing letters, memos, etc.
  • Bilingual, Spanish speaking preferred
