Evening Case Manager
Schedule: 3:00 p.m. – 11:30 p.m. - 5 per week (Monday through Friday)
Location: Eagle Star Housing – Transitional Housing Program
Position Summary:
Eagle Star Housing is seeking a dedicated, positive and compassionate Evening Case Manager to support the day-to-day operations of our 24/7 homeless shelter in Farmington. This role provides direct case management services to residents, working with individuals to overcome barriers such as income, credit, employment, mental health, and substance use, with the goal of helping clients successfully transition into permanent housing. You will also engage with residents, help prepare meals and engage in activities. This is not a nursing level of care, and all residents are fully independent.
We are looking for a leader on the evening shift - someone who has good judgement and can handle crisis situations calmly, has a good attitude and is a self-starter. We would love it if you had case management experience.
Key Responsibilities:
- Provide case management support, including intakes, service planning, and documentation.
- Work with Veterans and DSS clients to maintain eligibility and compliance with their cases.
- Support residents in addressing barriers to housing (income, employment, credit, mental health, substance use).
- Ensure resident safety, monitor adherence to house rules, and foster a positive living environment.
- Coordinate care with providers and referral sources to connect residents with needed services.
- Document incidents, case notes, and report concerns to the Housing Manager.
- Assist with household needs, which may include preparing dinner for up to 15 residents.
- Participate in required trainings and contribute as a collaborative team member.
Qualifications:
- Experience in case management, social work, or a related human services field preferred.
- Strong communication, problem-solving, and crisis intervention skills.
- Ability to maintain professional boundaries while supporting residents with diverse needs.
- Flexibility, dependability, and a commitment to Eagle Star Housing's mission.
- Computer Skills, including Microsoft Office and Teams.
