Job Title: Care Coordinator (CORE)

Job Code: SST012/Social Services Tech 3

Shift: Full-Time (40 hours per week on average)

Job Hours: Times May Vary

Base Location: Jonesboro, GA./Clayton Co.

Division/Department/Program: Clayton Opioid Response & Engagement (CORE)/Opioid Abatement Trust Program

This is a contract funded position with the Opioid Abatement Trust managed by the Department of Behavioral Health and Development Disabilities (DBHDD). The CORE Navigator will provide care coordination and resource development in Clayton County to individuals who are diagnosed or struggling with Opioid Use Disorder (OUD).

Duties & Responsibilities:

  • Ensure individuals encountered and served are referred to and enrolled in an appropriate OUD treatment program within one week of initial encounter.
  • Assess for SDOH (social determinants of health) for every individual served to ensure access and delivery of food stamps, Medicaid, and housing assistance.
  • Maintain a drug-free environment by providing regular drug screenings and support to help individuals stay drug-free throughout their recovery.
  • Collaborate with the Paula Crane Life Enrichment Center (PCLEC) to provide customized access in job training programs and secure meaningful job placements for at least 60% of individuals served.
  • Collaborate with the Paula Crane Life Enrichment Center (PCLEC) to assist individuals served completing their high school education or pursue higher education through GED programs, tutoring, and college application support.
  • Collaborate with Southside Medical (FQHC), Clayton Public Health District and Southern Regional Hospital to ensure regular health screenings, access to medical treatments are other essential integrated healthcare components.
  • Collaborate with Clayton County Police Department, specifically Clayton Center's Co-responder team and Clayton County Sheriff's Office to ensure individuals encountered during an OUD drug-related incident are effectively linked to OUD recovery services as determined by the individual's insurance coverage.
  • Collaborate with Clayton County Fire/EMS, specifically the 4C (Clayton County Community Cares) program to ensure individuals encountered during an OUD drug-related incident are effectively linked to OUD recovery services as determined by the individual's insurance coverage.
  • Collaborate with Clayton County Felony Drug Court and Mental Health/Resiliency Court to ensure effective and appropriate OUD recovery treatment via Clayton Center's Substance Abuse Intensive Outpatient Program (SAIOP) and/or the Paula Crane Life Enrichment Center (PCLEC). The utilization of the PCLEC program would involve peer-led recovery groups and peer recovery awareness and advocacy opportunities.
  • Other related duties as assigned.

Minimum Qualifications:

High school diploma/GED and three (3) years of job-related experience in a social services-related position; or one (1) year of experience at the lower-level Social Services Tech 2 (SST011) or equivalent position.

Preferred Qualifications:

  • Bachelor's degree in a related field from an accredited college or university.
  • Two (2) years of care coordination experience of at-risk populations.
  • Comfortable with working with individuals of diverse cultures, socio-economic statuses, those in under-resourced communities, etc.
  • Comfortable, engaging with non-traditional audiences in non-traditional settings; overall flexibility as to work environment, acuity, and needs of individuals/persons served.
  • Sensitivity to issues related to homelessness, mental illness, substance use, poverty, and social marginalization.
  • High level of oral, written, and technical communication.
  • Exceptional organizational, time management, and creative problem-solving ablity.
  • Understanding and appreciation of the Harm Reduction model and knowledge of the Opioid national crisis.
  • Capability to effectively work independently and collaboratively as part of a multi-disciplinary team.
  • Knowledge and understanding of (or ability to quickly develop) wide array of local resources available to individuals serve.
  • Knowledge and understanding of (or ability to quickly develop) Opioid Abatement Trust, DBHDD and Medicaid policies and procedures

Requirements/Competencies:

  • Must be able to lift 20 pounds.
  • Requires long periods of sitting/standing.
  • Must have valid Georgia driver's license and Motor Vehicle Record in accordance with company policy.
  • Candidates for selection must pass a criminal background check (including fingerprinting).
  • Must pass a pre-employment drug screen and subject to random drug screens.

Note: Some positions may require a valid driver's license.
