Primary Responsibilities:

  • Conduct comprehensive bio-psycho-socio-cultural assessments for patients and families receiving end-of-life care; create and implement therapeutic treatment plan.
  • Provides crisis intervention, individual, couple and group therapy to patients and families for a diverse range of issues related to grief/loss, adjustment to illness, relationships/family dynamics, mental health, abuse, symptom/pain management, spirituality and bereavement.
  • Facilitates case management with referral to appropriate community resources; promotes patient and family advocacy.
  • Provides counseling to patients and families regarding end-of-life care issues including medical decision making, Advanced Health Care Directives, disease process and caregiving; initiating community outreach.
  • Participates in the provision of social work services in a transdisciplinary care team setting; contributes/participates regularly in IDG team conferences and collaborates with team members in developing a patient centered plan of care.
  • Participates in regular clinical supervision with designated Clinical Social Worker and attends Accountability Training.
  • Assists in the training and orientation of new hires.
  • Performs other duties as required.

Knowledge and Skills Requirements:

  • Comprehensive theoretical and practical knowledge of individual, group, and family behavior.
  • Comprehensive knowledge of the following:
  • Ethical considerations involved in the practice of licensed social work; familiarity with NASW and NHPCO standards
  • Methods, principles, practices, and techniques of assessment, treatment planning and intervention, case management and group work, client-centered advocacy, community organization, research, psychotherapy, and other clinical skills as they apply to hospice patient and family population
  • Individual and group therapy techniques and theories
  • Social, emotional, and financial impact of various health issues on patients, their support system, and the community
  • Available community resources and their organization, purpose, and use
  • Possesses the ability to:
  • Apply a variety of techniques and methods in the areas of assessment, treatment planning, individual and group therapy, administration, and community organization
  • Analyze and evaluate the effectiveness of various social work treatment practices, techniques, and procedures; implement treatment plan based on patient and family needs
  • Establish and maintain effective and professional working relationships with Navian Hawaii staff, clients, family members, the public, and representatives of other agencies and organizations
  • Maintain clinical records and prepare related reports
  • Communicate effectively and professionally with all Navian Hawaii staff

Experience and Educational Requirements:

  • Masters Degree in Social Work (MSW) from an accredited school of social work
  • Possession of a current license to practice as a Licensed Social Worker (LSW) in the State of Hawaii
  • At least one year of experience in a related social work field

Hours:

  • Generally Monday through Friday, 8:00am through 4:30pm
  • However, position may require after hours work including weekends and holidays, as needed

Salary Range:

  • $75,000 To $85,000 Per Year
