Nurse Practitioner, Full-time

Qualifications

• Education: Master's degree in Nursing and completion of an accredited Nurse Practitioner program.
• Certification: Board Certified as Adult, Family, or Gerontology NP; Hospice and Palliative Care certification preferred.
• Experience: Minimum 3 years nursing experience with demonstrated expertise in pain and symptom management for palliative populations; prior experience with terminally ill patients preferred.

Skills:

• Strong ability to collaborate within an interdisciplinary team, including volunteers.
• Excellent communication and presentation skills for engaging physicians and the public.
• Proficient in Microsoft Word and Outlook.

Essential Duties

• Conduct comprehensive assessments, diagnose conditions, and develop individualized care plans in collaboration with the Medical Director.
• Provide symptom management and palliative interventions across multiple settings (home, hospital, clinic, long-term care).
• Complete hospice certification visits and participate in limited on-call rotation.
• Educate and support patients and families, ensuring clear communication of goals of care.
• Document assessments, interventions, and billing accurately and timely.
• Maintain compliance with quality standards, continuing education, and organizational policies.

Work Environment: The Palliative Care Nurse Practitioner provides care in diverse settings, including hospitals, palliative care clinics, skilled nursing facilities, assisted living communities, and long-term acute care centers. Typical schedule is Monday-Friday, 8:00 AM-4:00 PM, with limited on-call responsibilities (approximately once per quarter). Holiday coverage is shared equitably among team members.

Physical Demands: This position requires moderate to intense visual concentration for reading, computer use, and reviewing patient charts. There is a potential risk of exposure to infectious diseases, requiring adherence to standard precautions. The Nurse Practitioner must have the mobility to ambulate and the ability to lift or assist in lifting patients, with or without devices, in excess of 75 pounds, using proper body mechanics.
