About the RoleElmwood Adult Day Health Care Center is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our interdisciplinary team. In this role, you'll play a key part in ensuring the health and well-being of our participants through direct care, clinical support, and holistic, person-centered services.

Key Responsibilities:
Patient Care:
Administer medications and treatments in accordance with individual care plans.
- Conduct nursing assessments to monitor and evaluate participant health.
- Ensure prompt and effective care delivery tailored to participant needs.
- Maintain accurate and up-to-date medical records for all participants.
Assessment and Evaluation:
Conduct comprehensive assessments of patients' health status, including vital signs, medical history, and physical examinations.
- Develop and implement individualized care plans based on assessment findings and patient goals.
Care Coordination and Planning:
Assist in developing and updating individualized care plans.
- Conduct weekly care planning meetings in collaboration with the Nursing Director.
- Work with interdisciplinary team members to address changes in participant conditions.
- Collaborate with physicians, therapists, and other healthcare professionals to coordinate care and ensure a cohesive approach to patient management. Communicate effectively with the interdisciplinary team to address any changes in patients' conditions or needs.
Patient, Family and Staff Education and Communications:
Educate patients and their families on disease management, medication administration, self-care techniques, and lifestyle modifications.
- Provide guidance and support to help patients and families manage health conditions and enhance their quality of life by providing health literacy.
- Serve as a point of contact for staff and families regarding participant health and care.
- Foster open, respectful communication with participants, caregivers, and colleagues.
- Support staff in resolving participant care issues.
Documentation:
Maintain accurate, detailed, and timely documentation of patient care, including progress notes, treatment plans, and any changes in patients' conditions.
- Ensure documentation complies with company policies, insurance requirements, and regulatory standards.
Health, Safety, Confidentiality and Compliance:
Adhere to infection control practices, safety protocols, and all relevant regulations to ensure a safe care environment.
- Use medical equipment and supplies safely and appropriately.
- Maintain strict confidentiality of all participant and organizational information.
- Adhere to HIPAA and all regulatory guidelines related to healthcare documentation and privacy.
- Complies and upholds the policies and procedures of Elmwood Healthcare and it's local, state and federal regulatory authorities.
Emotional Support:
Provide compassionate support to patients and their families, addressing concerns and offering comfort and reassurance as needed.
- Foster a positive and supportive relationship with patients to encourage adherence to care plans.
Ongoing Support and Professional Development
Assist the Nursing Director and healthcare team in special projects or clinical tasks.
- Participate in ongoing training and professional development activities.

- Understanding of the unique healthcare needs of the elderly, especially those with cognitive impairments.

- Excellent communication, organizational, and problem-solving skills.
- Ability to work collaboratively within a multidisciplinary team.
- Support Certified Nursing Assistant (CNA) care.
