Overview
We are seeking a dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong foundation in patient care, with experience across various clinical settings including long-term care, assisted living, hospitals, and outpatient clinics. This role offers an opportunity to provide compassionate, high-quality nursing services while working within a dynamic healthcare environment that values professionalism and continuous learning. The LPN will be responsible for delivering direct patient care, assisting with medical procedures, and supporting interdisciplinary teams to promote optimal health outcomes.

Duties

  • Perform comprehensive patient assessments including vital signs, physical examinations, and health histories.
  • Administer medications and injections accurately following physician orders, including IV infusions and injections.
  • Conduct diagnostic evaluations such as specimen collection, processing, and basic laboratory testing.
  • Assist with complex procedures such as catheterization, wound suturing, and airway management.
  • Manage patient care plans including discharge planning, post-acute care coordination, and case management.
  • Utilize EMR/EHR systems like Epic, Cerner, Athenahealth, and eClinicalWorks for accurate documentation of medical records and clinical notes.
  • Provide specialized care in areas such as dementia care, Alzheimer's care, memory care, hospice care, and behavioral health.
  • Support patients in various settings including inpatient units such as ICU, PICU, NICU; outpatient clinics; home & community care; and urgent or emergency care environments.
  • Ensure infection control standards are maintained through aseptic techniques and adherence to HIPAA regulations.
  • Collaborate with multidisciplinary teams to develop individualized treatment plans for diverse patient populations including pediatrics, geriatrics, trauma patients (Level I & II Trauma Centers), and those with infectious diseases or chronic conditions.

Skills

  • Proficiency in medical terminology and anatomy/physiology knowledge necessary for accurate patient assessment and diagnosis.
  • Experience with EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks for efficient documentation.
  • Skilled in medication administration including IV insertion/insertion techniques and phlebotomy.
  • Strong understanding of infection control practices and sterile processing procedures.
  • Ability to perform physical examinations and diagnostic evaluations with attention to detail.
  • Competence in managing airway devices such as ventilators or tracheostomy tubes; experienced in advanced life support techniques like Basic Life Support (BLS).
  • Familiarity with specialized areas such as cardiac catheterization, spinal tap procedures, dermal fillers administration, or gastrostomy tube feeding is a plus.
  • Excellent communication skills for working with patients of all ages—including children—and collaborating effectively within healthcare teams.
  • Knowledge of ICD coding (ICD-9/10), CPT coding for billing purposes, and medical documentation standards aligned with NCQA standards is preferred.
  • Ability to work in fast-paced environments such as hospitals (including trauma centers), outpatient clinics, or home health settings while maintaining high-quality patient care.
