Job Overview
We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) specializing in hospice care to join our healthcare team. This role provides essential support to patients in their final stages of life, ensuring comfort, dignity, and personalized care. The ideal candidate will have a strong background in various healthcare settings, including long-term care, hospital environments, and specialized dementia or memory care. Experience with EMR/EHR systems and a thorough understanding of medical terminology and procedures are highly valued. Duties

  • Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary
  • Monitor and record vital signs accurately, including blood pressure, temperature, pulse, and respiration rates
  • Administer medications as prescribed under supervision, ensuring proper documentation in medical records
  • Provide emotional support and companionship to patients and their families during hospice care
  • Maintain infection control standards through aseptic techniques and proper use of PPE
  • Document patient care activities thoroughly within EMR/EHR systems while adhering to HIPAA regulations
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans
  • Support patients with special needs such as Alzheimer's/dementia care, memory care, behavioral health issues, or developmental disabilities
  • Assist with medical procedures such as catheter care or basic life support when required
  • Ensure a safe environment by identifying potential safety hazards and following protocols for infectious disease control

Experience

  • Prior experience in critical care settings such as ICU, emergency medicine, or Level I/II trauma centers is preferred
  • Experience working with vulnerable populations including geriatrics, pediatrics, behavioral health patients, or individuals with disabilities is highly desirable
  • Knowledge of EMR/EHR systems like Cerner or eClinicalWorks is advantageous
  • Demonstrated competence in medication administration, vital signs monitoring, and medical documentation
  • Background in hospice care or home & community-based care environments is beneficial
  • Certification as a CNA with current licensure required; additional certifications such as Basic Life Support (BLS) are preferred
  • Familiarity with specialized areas such as Alzheimer's/dementia/memory care or post-acute/sub-acute care settings enhances candidacy

This position is integral to delivering compassionate patient-centered care in hospice settings. We value candidates committed to excellence in healthcare delivery who can adapt to diverse clinical environments while maintaining the highest standards of safety and professionalism.

Pay: $45, $48,000.00 per year
