Position Summary
We are seeking a compassionate and motivated *Physical Therapist (PT)* to join our interdisciplinary care team in a *facility-based setting*. This role is ideal for *new graduates* or experienced therapists who are passionate about helping patients regain mobility, independence, and confidence through personalized rehabilitation plans.
You’ll work closely with residents, families, and clinical staff to deliver high-quality therapy services that improve functional outcomes and overall quality of life.
Key Responsibilities
* Evaluate patients and develop individualized treatment plans based on physician orders and patient goals
* Provide physical therapy services focused on strength, balance, mobility, gait training, and functional independence
* Educate patients and caregivers on exercises, safety techniques, and home programs
* Document evaluations, treatments, progress notes, and discharge summaries in accordance with regulatory standards
* Collaborate with nursing, therapy, and interdisciplinary teams to ensure coordinated care
* Monitor patient progress and adjust treatment plans as needed
* Maintain compliance with state practice acts, facility policies, and ethical standards
Qualifications
* Graduate of an accredited *Physical Therapy program*
* *Licensed or license-eligible* Physical Therapist in the state of practice
* *New graduates encouraged to apply*
* Strong communication and interpersonal skills

* Ability to work independently while thriving in a team environment

Job Types: Full-time, Part-time
Pay: $35.00 - $45.00 per hour
