Job Summary
We are seeking a dedicated and compassionate Social Worker to join our dynamic healthcare and community support team. In this vital role, you will provide comprehensive psychosocial assessments, develop individualized care plans, and deliver evidence-based interventions to diverse populations, including children, adults, and seniors. Duties
- Conduct thorough intake assessments and diagnostic evaluations utilizing medical terminology and HIPAA-compliant documentation practices.
- Develop personalized treatment plans incorporating psychotherapy modalities such as cognitive behavioral therapy (CBT), psychodynamic therapy, behavioral therapy, applied behavior analysis (ABA), and crisis intervention strategies.
- Provide individual, group, and family counseling sessions focused on mental health counseling, child & family counseling, addiction counseling, PTSD care, Alzheimer's care, and working with people with autism or disabilities.
- Utilize motivational interviewing techniques to foster client engagement and facilitate behavioral change.
- Coordinate with healthcare providers to ensure accurate medical records management using EMR systems while adhering to HIPAA regulations.
- Implement crisis management protocols during emergencies or acute mental health episodes.
- Engage in case management activities including patient assessment, service planning, resource linkage, and follow-up care in home or community settings.
- Collect data for clinical research projects related to behavioral health outcomes or social work interventions.
- Conduct diagnostic coding using ICD-9/10 codes for accurate documentation of conditions such as ICD coding for mental health disorders or ICD-10 for developmental disabilities.
- Collaborate with educators and specialists in special education programs when working with children with developmental or learning challenges.
- Participate in multidisciplinary team meetings to review patient progress and adjust treatment strategies accordingly.
- Support patients through culturally sensitive Christian counseling when appropriate.
Experience
- Proven experience in social work or behavioral health roles within hospital settings or community clinics.
- Demonstrated expertise in psychotherapy techniques including cognitive behavioral therapy (CBT), psychodynamic therapy, behavioral therapy, applied behavior analysis (ABA), and crisis intervention methods.
- Experience working with children with autism spectrum disorder (ASD), developmental disabilities, or special education needs is highly desirable.
- Familiarity with medical documentation standards including CPT coding for billing purposes and ICD coding for diagnoses.
- Hands-on experience utilizing electronic medical records (EMR) systems for documentation and data collection purposes.
- Knowledge of psychopharmacology principles relevant to mental health treatment plans is a plus.
- Strong understanding of HIPAA regulations ensuring confidentiality of patient information at all times.
- Ability to perform comprehensive patient assessments across diverse populations including geriatrics (Geriatrics) and individuals with chronic illnesses such as Alzheimer's disease or other neurodegenerative conditions.
