Unity Children's HomeCase Manager (Residential Treatment Services)

Schedule: Monday–Friday, 9:00 AM–5:00 PM
Compensation: Salaried — $50,000 semi-monthly
Location: Spring/Houston, TX

Position Summary

Unity Children's Home is seeking an experienced, highly skilled Case Manager to support youth placed in our Residential Treatment Center across four specialized treatment packages:

  • Complex Mental Health Disorders
  • Aggression & Defiant Behaviors
  • Human Trafficking Survivors / High-Risk Exploitation
  • Neurodevelopmental Disorders (Autism/IDD)
  • Additionally, youth may present with
  • substance use and co-occurring behavioral needs
  • .

This position is treatment-planning heavy and requires expertise in DFPS documentation standards, clinical assessments, service planning, and ongoing evaluation of youth progress. The Case Manager plays a central role in ensuring that treatment aligns with Unity's therapeutic model (CBT, DBT, MI, TBRI-informed environment) and meets all T3C and DFPS timelines.

Essential Responsibilities1. Treatment Planning & Clinical Documentation

  • Develop, update, and maintain individualized Treatment Plans, including goals, interventions, and measurable outcomes.
  • Complete admission assessments, mission assessments, psychosocial updates, and service reviews.
  • Conduct routine evaluations using UCH treatment tools (milieu worksheets, daily notes review, value evaluations, behavior level data, etc.).
  • Ensure all treatment documentation is completed timely and accurately in compliance with DFPS Minimum Standards and T3C guidelines.
  • Update treatment plans based on youth progress, assessments, and interdisciplinary team feedback.

2. Case Coordination & Youth Support

  • Meet with assigned youth regularly to discuss progress, barriers, goals, and treatment engagement.
  • Participate in and/or lead training modules, psychoeducational groups, and skill-building sessions as assigned.
  • Collaborate with clinical staff, supervisors, QMHPs, crisis team, and direct care staff to ensure consistent treatment implementation.
  • Coordinate services with external providers, psychiatric supports, educational teams, medical providers, and DFPS caseworkers.
  • Attend required meetings, treatment team conferences, and service plan reviews.

3. Compliance & Quality Assurance

  • Ensure all documentation meets agency, licensing, T3C, and DFPS requirements.
  • Monitor treatment deadlines, service provision timelines, and required contact frequency.
  • Maintain audit-ready charts (youth files and case documentation).
  • Support QA processes by participating in internal audits and corrective action steps.

4. Training & Staff Support

  • Participate in agency-wide training sessions and staff development.
  • Conduct training for direct care staff related to:
  • Treatment plan goals
  • Behavior strategies
  • Trauma-informed principles
  • Specialized population needs (HT, Autism/IDD, aggression, MH, substance use)
  • Support onboarding of new staff by explaining treatment models and documentation expectations.

5. Family, DFPS, and Stakeholder Collaboration

  • Maintain professional, timely communication with DFPS caseworkers, families, guardians, and placement specialists.
  • Provide updates regarding youth progress, crisis events, treatment needs, and discharge planning.
  • Participate in permanency planning, court-related documentation, and transition planning.

Required Qualifications

  • Master's degree or higher in Social Work, Counseling, Psychology, or a closely related field.
  • 3–5 years of experience working with DFPS-involved youth, residential programs, or high-acuity behavioral health populations.
  • Strong skills in developing and implementing treatment plans and documenting clinical interventions.
  • Knowledge of trauma-informed care, behavioral intervention models, crisis de-escalation, and therapeutic communication.
  • Ability to meet strict timelines and manage multiple cases simultaneously.
  • Strong written communication, organization, and computer documentation abilities.

Preferred Experience

  • Experience with high-acuity youth (human trafficking survivors, aggressive/defiant behaviors, complex MH, autism/IDD, and substance use).
  • Experience with DFPS T3C timelines, treatment service packages, and documentation models.
  • Familiarity with Extended Reach or similar case-management systems.
