Now Hiring: LPN Licensed Practical Nurse
Responsibilities:

  • Administer medication and perform treatments to residents accurately and safely in accordance with their individual care plan and physician's orders.
  • Perform treatments and procedures, as assigned, per physician's orders, strictly within the LPN scope of practice for the state of Wisconsin.
  • Accurately report and record the resident's condition and all provided care, paying close attention to signs and symptoms which may be indicative of a change in status, and communicate these changes to the Unit Manager/RN.
  • Participate in the development and revision of the interdisciplinary plan of care.
  • Ensure the well-being and satisfaction of residents is maintained throughout the shift.
  • Supervise certified nursing assistants (CNAs) and ensure their adherence to established policies and procedures.
  • Perform all duties in accordance with local, state, and federal guidelines, as well as the facility's policies and procedures.
  • Other duties as assigned by Unit Manager, Director of Nursing, and Administrator.

Qualifications:

  • Ability to work independently with minimal supervision, or as a productive and collaborative part of a team.
  • Basic computer knowledge for electronic medical record (EMR) charting and documentation.
  • Previous long-term care experience is preferred but not required—we welcome motivated new graduates
  • A compassionate, patient-centered approach to care.

License:

  • Must have an Active LPN Licensed Practical Nurse license in good standing in the state of Wisconsin.
