Position Summary

The Project Manager plays a critical role in coordinating efforts across departments within the Phoenix Children's Care Network (PCCN) to successfully facilitate highly complex network-related clinical and operational projects. This position ensures alignment between clinical, operational, and administrative initiatives—particularly those supporting PCCN's Integrated Care Coordination, Practice Integration, Quality, and Value-Based Care strategies.

The ideal candidate is a strategic thinker and strong communicator who can lead cross-functional teams, manage multiple priorities, and drive measurable improvements in patient outcomes, care coordination, quality metrics, and overall network performance.

Key Responsibilities

  • Lead and manage clinical and operational projects across PCCN from initiation through completion.
  • Collaborate with network leaders and stakeholders to ensure project goals align with organizational strategies.
  • Develop and maintain project plans, timelines, and deliverables using project management tools.
  • Facilitate communication and coordination among multidisciplinary teams to achieve project milestones.
  • Monitor project progress, identify risks, and develop mitigation strategies.
  • Report project outcomes, performance metrics, and improvement opportunities to executive leadership.

Required Qualifications
Education:

  • Bachelor's degree in Healthcare Administration, Business, Project Management, or related field.

Experience:

  • Minimum of 3 years of experience leading complex projects in a healthcare setting (preferably as a Project Manager).

Special Skills:

  • Exceptional interpersonal, facilitation, and problem-solving skills.
  • Proficiency with project management tools (e.g., Smartsheet, MS Project, Jira, Asana).

Preferred Qualifications
Certifications:

  • PMP (Project Management Professional) certification or equivalent preferred.

This position works in coordination with departments across the Phoenix Childrens Care Network to successfully facilitate highly complex network-related clinical and operational projects. The role ensures alignment between clinical, operational, and administrative initiatives—particularly those supporting PCCNs Integrated Care Coordination, Practice Integration, Quality, and value-based care strategies. This position requires excellent organization and strong interpersonal communication skills to lead cross-functional teams and manage projects that improve patient outcomes, care coordination, quality metrics, and overall network performance. This position reports to the Vice President, Network Operations and Clinical Programs.

Position Duties

  • Provides support for project intake, definition, and kick-off
  • Works with project stakeholders—including CIN leadership, ICC, PIT, and Quality teams—to help define project scope and objectives.
  • Helps create comprehensive Project Charters outlining deliverables, timelines, resource allocation, and alignment with CIN strategic goals.
  • Leads project teams to define clear roles for clinical, operational, and network program stakeholders.
  • Holds kick-off meetings to clearly and consistently outline project plans, ownership, and roles to ensure coordinated execution across all relevant programs.

  • Maintains comprehensive documentation of all projects throughout the project cycle

  • Monitors and updates all projects in the project management process, ensuring inclusion of network operational and quality-related project data.
  • Creates or oversees all project communications to ensure consistent updates are provided to leadership, program managers, and other stakeholders.
  • Tracks issues and risks during project implementation to ensure projects are completed within scope, on time, and in compliance with privacy/security regulations (e.g., HIPAA).
  • Provides guidance to project owners ensuring regular meetings, reporting, and stakeholder communications occur.

  • Manages highly complex projects in coordination with the project team

  • Defines project scope, plan, budget, and schedule within each Project Charter, ensuring alignment with CIN, ICC, PIT, and quality program objectives.
  • Leads multidisciplinary teams—including data and analytics, clinical, operational and administrative stakeholders—through successful project execution.
  • Schedules and documents regular meetings to ensure tracking toward goals such as improved quality performance, care coordination, provider engagement, and practice integration.
  • Tracks issues, risks, dependencies, and mitigation steps, and communicates progress to leadership, providers, and committees.

  • Consistently demonstrates initiative and accountability

  • Works independently to manage multiple concurrent projects with minimal guidance, including those impacting population health, quality metrics, and value-based care outcomes.
  • Identifies and communicates all project risks, assigning resolution responsibilities and ensuring timely mitigation.
  • Collaborates with project members' leadership to ensure accountability for task completion and alignment with CIN strategic priorities.
  • Serves as a liaison between clinical programs, network leadership and other appropriate stakeholders to resolve barriers, foster engagement, and support change management initiatives.

  • Supports CIN-related committees, reporting, and continuous improvement

  • Prepares agendas, facilitates meetings, and documents decisions for CIN committees, workgroups, and steering teams.
  • Partners with analysts to define data/reporting needs, including dashboards for quality measures, provider engagement, and program performance.
  • Incorporates feedback from stakeholders into continuous improvement efforts for workflows, systems, and care models.

  • Performs miscellaneous job related duties as requested.
