Position Summary:
We are currently seeking a dedicated and experienced Case Manager to join our team. The ideal candidate will have strong counseling and case management experience, a CADC or RADT certification. This role requires compassion, organization, and the ability to thrive in a fast-paced clinical environment.
Key Responsibilities:
- Responsible for completion of pre-screening assessments for all potential clients.
- Responsible for facilitating the admission and intake process.
- Responsible for centralized communications regarding information on program and services.
- Responds to request from the clients, vendors, families, other treatment facilities, and healthcare providers.
- Ensures immediate response to all inquiries made and follows through on admission or otherwise provides appropriate referrals for alternative treatment.
- Facilitates the admission and intake process, inclusive of funding payments for treatment services prior to admission
- Gathers all pertinent clinical information of potential clients to begin admission process and treatment planning. Works directly with, Program Director, and Clinical Director to ensure smooth admission process.
- Able to work with little direct supervision, able to handle "difficult" clients and works well under pressure.
- Able to organize time efficiently and follows through on all calls to an appropriate resolution.
- Will coordinate and schedule site visits for potential admissions as well as other sources for further business development.
- Assists with education and information to inquiries for provision of support and intervention.
- Oversees alumni – making post discharge contacts, facilities alumni meetings/events
- Maintain outcomes database
- Initiate discharge plans for all client – aftercare and linkage
- Maintain all CPR/First Aid certifications as required
- Provides appropriate staff development and training opportunities to staff to ensure a high level of technical skills and knowledge of mental health issues.
- Regularly audits charts and other client-related documentation related to the treatment of clients.
- Intervenes as an advocate for clients to resolve emergency problems in crisis situations.
- Attends training sessions to increase knowledge and skills.
Qualifications:
- Required: CADC, CADC-II, or RADT certification
- Required: 2+ year experience in case management and counseling in a behavioral health or substance use disorder setting
- Excellent communication, organizational, and interpersonal skills
- Must pass a criminal background check
