About Qualicare SF Bay

Our San Ramon-based team is growing, and we're looking for a Care Coordinator Manager to play a key role in ensuring that our clients receive seamless, personalized care while supporting our caregivers and clinical staff every step of the way.

Position Summary

The Care Coordinator plays a central role in ensuring high-quality, client-centered home care services. This position is responsible for coordinating caregiver schedules, managing client care plans, communicating with families, and serving as the primary liaison between clients, caregivers, and the home care agency. The Care Coordinator ensures smooth daily operations, timely staffing, and a positive client experience.

Key Responsibilities
1. Client Care Management

  • Coordinate intake process for new clients, including assessments, documentation, and creation of care plans.
  • Collaborate with nurses, care managers, and families to ensure that care plans reflect client needs and preferences.
  • Monitor client satisfaction and address concerns promptly to maintain quality of care.
  • Update care plans as client conditions or needs change.

2. Scheduling & Staffing

  • Match caregivers with clients based on skills, experience, and availability.
  • Maintain and adjust caregiver schedules to ensure coverage at all times.
  • Respond to last-minute schedule changes, call-outs, and urgent staffing needs.
  • Ensure caregivers receive orientation, instructions, and ongoing support for client assignments.

3. Communication & Relationship Building

  • Serve as the main point of contact for clients and families regarding scheduling and service questions.
  • Provide regular updates to families on client well-being and care progress.
  • Build positive, professional relationships with caregivers to support retention and engagement.

4. Administrative & Compliance

  • Maintain accurate client and caregiver records in scheduling in Wellsky.
  • Ensure compliance with state regulations, agency policies, and documentation standards.
  • Assist with caregiver onboarding, credentialing, and training coordination.
  • Track and report key metrics, including client satisfaction, missed shifts, and service hours.

Qualifications

  • Education: High school diploma or equivalent required; Associate's or Bachelor's in healthcare administration, nursing, or social services preferred.
  • Experience:
  • 1–2 years in home care, healthcare, case management, or scheduling/coordination role.
  • Experience with caregiver scheduling systems (ClearCare/Alora/MatrixCare) a plus.
  • Skills:
  • Strong organizational and time management abilities.
  • Excellent communication and problem-solving skills.
  • Ability to multitask in a fast-paced environment.
  • Proficiency with Microsoft Office and scheduling software.
