Job Overview
We are seeking a compassionate and dedicated Hospice Nurse to join our team, providing exceptional end-of-life care that emphasizes comfort, dignity, and personalized support. As a vital member of our hospice care program, you will deliver holistic nursing services to patients in their homes or care facilities, ensuring their physical, emotional, and spiritual needs are met with empathy and professionalism. Responsibilities
- Conduct comprehensive patient assessments, including vital signs, symptom management, and physiological evaluations to develop tailored care plans.
- Administer medications and treatments such as IV infusions, pain management protocols, and medication administration in accordance with physician orders and safety standards.
- Provide emotional support and education to patients and their families regarding disease progression, symptom control, and comfort measures.
- Coordinate with interdisciplinary teams to ensure seamless delivery of hospice services, including collaboration with physicians, social workers, chaplains, and therapists.
- Monitor patient progress regularly through documentation of medical records using EMR (Electronic Medical Records) systems like Epic or Cerner; ensure compliance with HIPAA regulations.
- Manage complex symptoms such as acute pain or respiratory distress using aseptic techniques and advanced nursing skills like airway management or tube feeding.
- Educate caregivers on wound care, medication administration, infection control practices, and other essential aspects of home-based hospice care.
Experience
- Proven experience in hospice & palliative medicine or related fields such as critical care, ICU, emergency medicine, or medical-surgical nursing.
- Home Hospice Care
- Demonstrated proficiency with EMR/EHR systems including Axxess, Wellsky, Epic, Cerner, or Athenahealth for accurate documentation.
- Skilled in performing physical examinations, patient assessment, IV insertion, catheter care, and medication administration.
- Familiarity with diverse healthcare settings such as hospitals (Level I & II trauma centers), nursing homes, assisted living facilities, or home care environments.
- Knowledge of infectious disease care protocols, pain management strategies, trauma medicine principles, and end-of-life care standards aligned with NCQA standards.
- Experience working with special populations including pediatrics (neonatal ventilator), geriatrics (memory care), dementia or Alzheimer's care, developmental disabilities, or behavioral health needs.
- Ability to handle complex cases involving acute pain management, terminal illness support, discharge planning, and coordination of long-term care services.
