Overview
Join our compassionate team as a Hospice Social Worker, where you will play a vital role in providing emotional support, counseling, and comprehensive case management to patients and their families facing life-limiting illnesses. This dynamic position offers an opportunity to make a meaningful difference by delivering holistic, patient-centered care in a variety of settings including home, inpatient facilities, nursing homes, and outpatient clinics. Your expertise will help ease emotional distress, facilitate advance care planning, and ensure patients' social and psychological needs are met with dignity and respect.

Duties

  • Conduct psychosocial assessments for patients and families.
  • Develop individualized care plans in collaboration with interdisciplinary teams.
  • Provide counseling and emotional support for patients and families coping with terminal illness.
  • Facilitate advance care planning (living wills, healthcare directives).
  • Connect families with community resources (financial aid, housing, support groups).
  • Deliver bereavement support before and after patient death.
  • Ensure compliance with Medicaid, Medicare, and hospice regulations.
  • Document patient progress and maintain HIPAA-compliant records.
  • Collaborate with physicians, nurses, chaplains, and emergency preparedness coordinators to optimize patient experience.

Skills

  • Education: Master's degree in Social Work (MSW) preferred; Bachelor's degree considered with relevant experience.
  • Licensure: Current Michigan Social Work license required.
  • Experience: 2+ years in hospice, palliative care, or healthcare social work preferred.
  • Skills: Empathy, crisis intervention, communication, and regulatory awareness.
  • Bonus: Familiarity with payer-aligned pilots, RPM/CCM workflows, and coalition-building for compliance acceleration.
  • Extensive experience in hospice care with a strong understanding of end-of-life issues and dementia or Alzheimer's disease care.
  • Proficiency in child & family counseling, mental health counseling, crisis management, and working with people with disabilities or developmental disabilities.
  • Knowledge of medical terminology, ICD coding systems (ICD-9/10), CPT coding, and medical records management.
  • Skilled in behavioral therapy modalities such as cognitive behavioral therapy (CBT), applied behavior analysis (ABA), and trauma-informed approaches like PTSD care.
  • Demonstrated ability to perform diagnostic evaluations and mental health assessments within diverse populations including pediatrics, geriatrics, and special education settings.
  • Experience working with EMR systems for data collection and documentation; familiarity with HIPAA compliance standards.
  • Strong case management skills with the ability to coordinate services across multiple healthcare settings including outpatient clinics and inpatient units.
  • Excellent communication skills for intake interviews, patient education, family counseling sessions, and interdisciplinary team collaboration.
  • Capable of crisis intervention in high-stress situations involving terminal illness or behavioral health crises; adept at working with hospice teams to provide compassionate support. Pay: From $53,835.46 per year
