Licensed Practical Nurse

Job Overview
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will utilize their expertise in medical documentation, patient assessment, medication administration, and vital signs monitoring to support patient recovery and well-being.

Responsibilities

• Provide direct patient care and assist with daily living activities in clients' homes.
• Administer medications, injections, and treatments as prescribed.
• Monitor patient vital signs and report changes in condition to supervising RN or physician.
• Assist with wound care, catheter care, and mobility support as needed.
• Document patient care, observations, and progress with accuracy.
• Educate patients and families regarding health management and safety protocols.
• Maintain compliance with EEOICPA standards, HIPAA policies, and company procedures.
• Collaborate with caregiving team to ensure exceptional service and continuity of care.
• Respond promptly and professionally to patient needs and emergencies.

Requirements

• Valid and active LPN license in South Carolina.
• Minimum of one year of clinical experience (home health, med-surg, or hospice preferred).
• Knowledge of best practices in patient care and safety.
• Ability to work independently, demonstrate initiative, and adapt to changing patient needs.
• Excellent communication, organizational, and documentation skills.
• Current CPR certification (required).
• Ability to travel to patient residences within service area.
• Experience working with EEOICPA or occupational illness patients preferred, but not required.

Pay: $35.00 per hour

Expected hours: 12.0 – 36.0 per week
