Overview
CREATE Independence is a nonprofit life skills and transitional services organization based in Weatogue, Connecticut. We support adolescents and young adults with varying abilities as they develop independence, build vocational and daily living skills, and engage meaningfully with their communities. Our person-centered programs are rooted in respect, empowerment, and inclusion.
We are seeking a caring and dedicated professional to provide individualized, 1:1 support for a participant in our Transition Program. This role is focused on helping the individual achieve their personal goals in employment readiness, community engagement, social development, and independent living. The ideal candidate will be patient, resourceful, and committed to building a positive, empowering relationship within a structured program setting.
Key Responsibilities
Individualized Transition Support
- Provide consistent 1:1 assistance to a designated participant throughout Transition Program hours.
- Support the participant in developing skills related to job readiness, community access, and independent living.
- Adapt activities and teaching strategies to align with the participant's learning style, interests, and needs.
- Assist with daily activities, including job readiness tasks, life skills lessons, and educational activities.
Skill Development & Instruction
- Coach the participant in social interaction, vocational tasks, and daily living skills.
- Facilitate meaningful participation in program activities and community outings.
- Provide guidance in using assistive technology or accommodations when appropriate.
- Promote a safe, respectful, and supportive environment at all times.
Monitoring & Documentation
- Track and document progress toward individualized transition goals.
- Provide regular updates to the Head of the Transition program and Director on strengths, challenges, and recommendations.
- Maintain accurate and timely records of activities, progress, and any incidents.
- Follow all program, health, and safety policies and procedures.
Collaboration & Communication
- Work closely with Transition Program staff, vocational services, and other support team members to ensure coordinated care.
- Communicate effectively with families and relevant stakeholders regarding participant progress.
- Participate in planning meetings to share insights and adapt support strategies as needed.
Qualifications
- 1-2 years of experience working with individuals with disabilities in a school, transition program, or similar setting.
- Strong interpersonal and communication skills.
- Ability to adapt to changing needs and respond effectively in real time.
- Reliable transportation and ability to accompany the participant to activities.
- CPR/First Aid certification preferred or willingness to obtain.
- Patience, empathy, and a commitment to person-centered support.
- Successful completion of a background check is required.
Working Conditions
- Environment: Work is performed in a variety of settings, including the day program facility and community locations.
- Physical Demands: The role may require lifting, standing, and assisting participants with physical activities.
Pay: $ $23.00 per hour
