Guardianship Case Manager

Job Description:

You will provide guardianship responsibilities to at risk adults throughout the Denver area. Reporting directly to the GAC Lead Project Manager, you will be required to act as medical power of attorney, advocate in safeguarding client's rights, address client concerns, assist in coordinating appointments and services, and conduct site visits throughout Colorado. You will have the opportunity to work with at risk adults who are experiencing major challenges such as Severe mental illness, Dementia/Alzheimer's, and Developmental disabilities. The Client Manager ensures that the integrity and respect of every client are upheld while addressing their personal needs. A person qualified to be a Case Manager is well organized and works well as a member of a dedicated team.

Job Responsibilities:

  • Make mandatory home visits with our clients on a regular basis.
  • Maintains a caseload of up to 25 clients.
  • includes travel
  • Determines and monitors the residence of the protected person.
  • Addresses and responds to client concerns regarding placement, personal needs, and medical needs.
  • Advocates for clients to ensure they are treated with respect and dignity, free of discrimination.
  • Ensures client is enrolled and has access to all available insurance, programs, and benefits.
  • Consents to and monitors medical treatment.
  • Consents to and monitors non-medical services such as education and counseling.
  • Makes end-of-life decisions on behalf of the protected person.
  • Maximizes the independence of the protected person in the least restrictive manner.
  • Reports to the court about the guardianship status annually.
  • Other duties as assigned by supervisor.

Qualifications:

  • This position requires travel.
  • AA in Social or Human Services or related field
  • at least 1 year of experience in human services, social work, criminal justice, mental health, healthcare
  • Knowledge of guardianship and probate law
  • Bilingual Spanish is preferred, but not required.
