Job Summary:

JET Healthcare is seeking a dedicated, experienced, and enthusiastic Nurse Practitioner to join our growing team of healthcare professionals. The ideal candidate will have a passion for providing patient-centered care to seniors and will work collaboratively with our lead NP, Heather Hartman, and clinical team. This role is ideal for a self-motivated practitioner who values both independence and teamwork in delivering exceptional care to the elderly population in various settings, including senior homes, assisted living, skilled nursing, memory care, and independent living facilities.

Primary Duties and Responsibilities:

• Conduct comprehensive assessments and develop individualized care plans for elderly patients
• Diagnose and treat acute and chronic health conditions, collaborating closely with patients, families, and facility staff
• Manage patient medications, including prescribing and adjusting as needed
• Monitor patient progress and adjust care plans
• Educate patients and families about health conditions and effective care strategies
• Provide preventive care and health screenings to enhance patient quality of life
• Maintain thorough, accurate patient records and ensure compliance with regulatory standards
• Serve as a liaison between healthcare providers, patients, and families to ensure effective communication and care continuity
• Engage in continuous education and development to stay current with geriatric best practices and innovations

Qualifications:

• Master's or Doctoral degree in Nursing with a current Nurse Practitioner license
• Minimum of 1-2 years of experience working in geriatric, palliative, or adult care
• Familiarity with assisted living, skilled nursing, and senior care settings
• Excellent interpersonal skills, including communication, empathy, and patience
• Strong organizational skills and ability to manage a diverse caseload
• Commitment to delivering high-quality, patient-centered care to the elderly community

Locations:

• Decatur, IL and surrounding areas
• Assisted Living Facilities, Memory Care Units, and senior home patients
