Position Overview:

The Truancy Case Manager is responsible for being an ambassador of GPCA's mission, values, and commitment as they contribute to partnerships in an array of community settings. With the approach of working towards increasing organizational stability, breaking the cycle of intergenerational poverty, and having a greater impact on generational poverty the CM's will assist students and families to ensure household stability.

Under the supervision of Truancy Leadership, the Case Manager will collaborate with students and their families to improve student attendance and enhance educational outcomes. Duties include outreach to expand awareness of agency programs through the following: home visits as well as office, phone and written communication with clients; school visits to meet with staff, students and retrieve school documents; make appropriate referrals for clients; represent families at Regional Truancy Court and Family Court; coordinate with other agencies to assist clients; maintain case files including data entry, case notes.

Qualifications:

  • Bachelor's Degree in Social Work or Human Services field (required)
  • 2+ years of social service experience or 1+ year of experience plus 1+ year of internship
  • Working understanding of child and family development
  • Working understanding of community and resource development
  • Ability to work collaboratively in a multi-disciplined environment
  • Excellent written and verbal communications skills
  • Creative, outgoing team player with the ability to adapt quickly to changing needs and priorities
  • Strong time management skills
  • Ability to work with a diverse population
  • Must be detailed oriented and highly organized
  • Proficiency in MS Office Applications (Word, Outlook
  • Working knowledge of database applications
  • Willingness to travel approximately 50% of the time
  • Bilingual preferred (not required)

Key Responsibilities:

  • Represent GPCA's mission, values, and commitment in all Community settings
  • Adhere to the agencies Policies & Employee Handbook, Truancy Program Standards, and professional code of ethics
  • Serve a caseload of 35+ students age 5-18 residing in Philadelphia who have been referred to the Truancy Court system (Tier Two) and/or are at risk of legal truancy (Early Intervention) and their families.
  • Conduct a minimum of one successful home visit per month with each child and their family (two during summer) for court-involved families.
  • Conduct weekly contact in the form of call, email, text, or home visit (as needed) for Early Intervention cases.
  • School visit monthly during school year (weekly for Early Intervention referrals)
  • Document case progress in City Span Database (and Attendance Collaborative Worksheet for Early Intervention) weekly, and upload all required documentation on a monthly basis at minimum.
  • Prepare and submit documentation for Court in the form of Court Reports and FAST Assessments if needed.
  • Attend Regional Truancy Court to support client and family (Court involved only).
  • Collaborate with internal and external partners to assist families with overcoming barriers to attendance.
  • Collaborate with DHS and CUA as needed for families referred to the Family Court System (Tier Three).
  • Maintain case file as required by quality assurance.
  • Adhere to agency attendance policy and communicate PTO usage request to CS Supervisor effectively and timely
  • Document daily activities on Outlook calendar and share calendar with Supervisor
  • Immediately conference any child safety concerns with Truancy Supervisor for assessment and make report to Child line state reporting agency as needed
  • Other tasks as required by funder or agency.
