Sunrise Skilled Nursing & Rehab is offering an exciting opportunity for a dedicated and compassionate Memory Care Program Coordinator (CNA) to step into a leadership role within our specialized Memory Care Program. This full-time position is perfect for a Certified Nursing Assistant who is passionate about dementia care, enjoys mentoring others, and wants to advance into a higher-level care coordination role.

• Provide compassionate care while upholding the dignity and respect of every resident
• Assist with the development and presentation of quarterly wellness and care plans
• Support residents and families in maintaining physical, emotional, and cognitive well-being
• Deliver restorative nursing services integrated into each resident's daily care
• Coordinate all aspects of resident life in the Memory Care Unit, including admission evaluations and individualized care programming
• Ensure compliance with dementia care standards and provide support and guidance to staff
• Attend ongoing in-service training sessions to enhance dementia-specific and clinical knowledge
• Collaborate effectively with nursing and interdisciplinary departments to ensure continuity of care

• Certified Nursing Assistant (CNA) in good standing with the State of Illinois
• Previous long-term care or skilled nursing experience preferred
• Experience in dementia or memory care preferred (training available)
• Strong attendance, communication skills, and professional work ethic
