Franciscan Villa is looking for a skilled RN MDS Coordinator to join our team. Responsibilities:

• Conduct and complete resident assessments (MDS) accurately and on time
• Collaborate with the care team to develop individualized care plans
• Monitor and document resident progress
• Participate in Quality Assurance and Performance Improvement (QAPI)
• Communicate with residents, families, and healthcare providers
• Stay current on MDS regulations and guidelines

Requirements:

• RN license and RAC-CT certification in good standings with no restrictions
• Knowledge of MDS 3.0 and care planning
