Role Description: Case Manager at Rising Tide Community Initiatives Inc.
Position Overview: The Case Manager at Rising Tide Community Initiatives plays a pivotal role in supporting individuals facing homelessness as they transition into stable housing. The Case Manager is responsible for working directly with clients to develop and implement personalized case plans that address their unique needs and challenges, including mental health, polysubstance use, and other barriers to stability. This role requires a compassionate, culturally sensitive approach and the ability to engage with clients from diverse racial, ethnic, and socio-economic backgrounds. The Case Manager must perform their duties with a strong commitment to Rising Tide's mission and values, ensuring that all interactions are grounded in respect, empathy, and professionalism.
Qualifications, Training, and Experience
- Education: Bachelor of Social Work (BSW), Human Services Counsellor Diploma, or equivalent combination of education and experience in human, social, or healthcare services.
- Experience: Minimum of 1 year in a frontline case management role serving vulnerable populations.
- Training: Certification in Non-Violent Crisis Intervention and Applied Suicide Intervention Skills Training (ASIST) is required. If not already certified, the successful candidate must complete training at the earliest opportunity.

- Strong interest and ability to work with individuals from diverse backgrounds, including those experiencing homelessness, mental health challenges, and substance use disorders.
- In-depth knowledge of harm reduction and trauma-informed care practices.
- Critical thinking and analytical skills to assess and respond to complex client needs.
- Excellent communication and interpersonal skills, with the ability to build rapport and maintain professional boundaries.
- Ability to work effectively both independently and as part of a multidisciplinary team.
- Resilience and the ability to cope with stressful situations, including crisis response.
- Strong documentation skills and attention to detail.
Key Responsibilities:
- Case Planning and Client Support:
- Collaborate with clients to create individualized case plans that outline goals, strategies, and steps toward achieving housing stability and personal growth.
- Regularly assess clients' progress and adjust case plans as needed to address evolving needs and circumstances.
- Conduct home visits and community visits to monitor client progress, provide support, and build strong, trusting relationships.
- Apply critical thinking and analytical skills to assess clients' behavior, mental health status, and polysubstance use, using this information to guide case management decisions.
- Coordination and Collaboration:
- Work closely with a network of community partners, including social services, healthcare providers, addiction services, and governmental agencies, to coordinate comprehensive support for clients.
- Act as a liaison between clients and external resources, advocating for clients' needs and ensuring they have access to necessary services.
- Participate in multidisciplinary team meetings with other Rising Tide staff, including Housing Support Workers and Peer Support Workers, to provide holistic support to clients.
- Crisis Management and Intervention:
- Respond to crisis situations involving clients, including mental health emergencies, substance use incidents, and other urgent needs.
- Utilize non-violent crisis intervention techniques and apply a trauma-informed approach to de-escalate situations and support clients effectively.
- Be prepared to work in high-stress environments and cope with the challenges that arise in crisis situations.
- Documentation and Compliance:
- Maintain accurate and up-to-date records of all client interactions, assessments, and progress notes, ensuring compliance with Rising Tide's documentation standards.
- Document case plans, updates, and any incidents thoroughly and promptly to provide clear and comprehensive records of client care.
- Ensure confidentiality and privacy of all client information in accordance with legal and organizational guidelines.
- Application of Best Practices:
- Employ harm reduction techniques in working with clients who are experiencing polysubstance use, recognizing that each individual's path to recovery is unique.
- Apply trauma-informed care principles in all client interactions, understanding the impact of past trauma on current behavior and choices.
- Demonstrate cultural competency and sensitivity in all aspects of case management, ensuring that services are accessible and respectful of clients' diverse backgrounds.
- Independent and Team Work:
- Work independently to manage a caseload of clients, demonstrating strong organizational skills and self-motivation.
- Collaborate effectively with the broader Rising Tide team, sharing insights and participating in team efforts to enhance client outcomes.
- Be flexible and adaptable, willing to adjust work hours, including evenings and weekends, to meet the needs of clients.
Working Conditions:
- The Case Manager role involves working in both office and community settings, including conducting home visits.
