Skills of Central PA is seeking a passionate and experienced Licensed Practical Nurse (LPN) Supervisor to join our mission-driven team supporting individuals with intellectual and developmental disabilities (IDD). This is an exciting leadership opportunity for an LPN who is ready to grow their career while making a real difference across Centre, Clinton, Lycoming, Huntingdon, Mifflin, and Juniata Counties.
Based out of State College or Lewistown, PA, this role offers the chance to mentor and guide a team of LPNs, collaborate with interdisciplinary partners, and play a key role in ensuring high-quality, person-centered care. *Responsibilities:*
Our LPNs provide a day-to-day variety of nursing services and care to the people we support by using and maintaining established nursing objectives and standards, adhering to current nursing law. They Implement and evaluate preventive treatments respecting the rights of the people we support. Candidate must be flexible, a team player and able to take direction and follow through on the coordination of medical care for the folks that we support. The ideal candidate will be a proven leader and supervisor to oversee the LPNs in the geographical region. Effective written and verbal communications skills are necessary to be successful in this role; administrative experience preferred.
* Maintain a high level of accountability and dependability to include attendance at work and punctuality providing direct care when needed.
* Monitor EMR to ensure quality, Review Health Risk Screening Tools.
* Monitor quality of health care within the Community Residential and Lifesharing Programs.
* Be patient with others, including people with dementia or Alzheimer's disease and those who are verbally aggressive or difficult to control.
* Communicate clearly with doctors, other nurses, patients, family members, direct support professionals, and other personnel to ensure a proper care plan.
* Able to perform all Essential Duties and Responsibilities of a Licensed Practical Nurse.
This position is based on a standard 8A-4P work week (M-F); however, nurses (including the supervisor) are required to be on a call rotation for the residential homes they support including weekends, evenings, and holidays. Our LPNs have also been providing direct support, at times, in the residential homes to assist the Direct Support Professionals.
*Qualifications: Education & Experience:*
The Nurse Supervisor shall have one of the following groups of qualifications:
* High School diploma or GED equivalent and be a currently Licensed Practical Nurse with 2-years work experience working directly with individuals with an intellectual disability and at least 2 years of supervisory experience in a related field OR
* High School diploma or GED equivalent and be a currently Licensed Practical Nurse with 3-years work experience working directly with individuals with an intellectual disability and at least 1 year of supervisory experience in a related field OR
* High School diploma or GED equivalent and be a currently Licensed Practical Nurse with 4-years work experience working directly with individuals with an intellectual disability.

Pay: $60,320.00 - $74,048.00 per year
