We are seeking a skilled and compassionate *Home Health Physical Therapist* to provide in-home therapy services to patients based on physician orders. You will perform assessments, develop treatment plans, deliver therapeutic interventions, and help patients regain mobility, strength, and independence in their home environment.
*Compensation:* _Starting rate of $85 per visit_
*Service Areas:*
Casper, Wyoming
*Key Responsibilities*
* Complete initial and ongoing patient assessments, including OASIS assessments.
* Develop and update individualized treatment plans with physician approval.
* Provide therapeutic interventions to reduce pain and restore functional mobility (exercise, heat, massage, modalities, etc.).
* Educate and train patients and caregivers on treatment programs, mobility techniques, equipment use, and safety.
* Evaluate the home environment and family support to determine care needs.
* Document assessments, progress notes, and patient responses within required timeframes.
* Communicate patient status and changes to the physician and care team regularly.
* Supervise Physical Therapy Assistants and paraprofessionals per policy.
* Participate in case conferences, staff meetings, and discharge planning.
* Arrange outpatient services when care cannot be provided at home.
*Job Conditions*
* Must be able to drive and access homes that may not be wheelchair accessible.
* Requires adequate hearing, vision, and physical ability for patient assessments.
* Occasional bending, reaching, and assisting with patient movement (up to 250 lbs).
* Strong verbal and written communication skills in English required.
*Qualifications*
* Graduate of an accredited Physical Therapy program (APTA/AMA approved).
* Active state PT license.
* Minimum *1 year* of home health or institutional experience.
* Current *CPR certification*.
* Clean criminal background check.
* Not excluded from Medicare or Medicaid programs.
