At *Quality Care Home Health & Hospice*, we are committed to empowering patients to live more independently and comfortably. We are looking for a skilled *Physical Therapist (PT)* to join our team on a per-diem, part-time, or full-time basis, offering you the flexibility to make a significant impact on patients in both hospice and home care settings.
Your Role and Responsibilities:
As a Physical Therapist, you will:
* *Assess and Evaluate*: Conduct thorough evaluations of patients' muscle strength, mobility, gait, range of motion, and potential for rehabilitation.
* *Develop Care Plans*: Create personalized treatment plans in coordination with physicians and the care team, tailored to the needs of each patient.
* *Deliver Therapy*: Provide direct physical therapy treatments and guide patients in the use of prosthetic devices and therapeutic appliances.
* *Instruct and Educate*: Teach patients and their families/caregivers about exercises, treatment goals, and therapeutic techniques.
* *Support Home Care*: Offer specialized care and consultation for patients in their home environments, helping them regain function and independence.
* *Document and Report*: Maintain accurate clinical records, report patient progress, and participate in discharge planning.
* *Supervise and Coordinate*: Supervise physical therapy assistants and collaborate with other team members to ensure the highest quality of care.
What We’re Looking For:
We seek a Physical Therapist who is passionate about improving patients' quality of life and has the following qualifications:
* A degree in physical therapy from an accredited program.
* A valid license to practice as a Physical Therapist in the state.
* At least two years of clinical experience as a physical therapist, with home health or hospice experience preferred.
* Strong communication, organizational, and documentation skills.
* Current CPR certification.
* A valid driver’s license and access to a reliable, insured vehicle.

Pay: $45.00 - $50.00 per hour
