*Signature Healthcare at Home *is seeking compassionate *Physical Therapists to provide Start of Care (SOC) coverage* throughout Albany, Corvallis, and Sweet Home. Enjoy flexible scheduling and seeing patients close to home!
*$5,000 retention bonus full-time only*
*Home Health Physical Therapist*
* *Territory: *Albany, Corvallis, Lebanon, and Sweet Home
* *Type*: Consistent PRN, part-time or Full-time
* *Work Schedule: *day shift- Tuesday-Saturday, Monday-Friday or Sunday-Thursday. Flexible hours
* $47-$55
*Responsibilities*
* Conduct comprehensive evaluations of patients to determine their physical therapy needs.
* Develop individualized treatment plans based on patient assessments and goals.
* Implement therapeutic interventions including but not limited to trigger point therapy, orthopedic rehabilitation, and acute care strategies.
* Educate patients and their families about treatment plans, exercises, and self-care techniques.
* Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes.
* Maintain accurate documentation of patient treatments, progress notes, and discharge summaries.
* Collaborate with other healthcare professionals to provide comprehensive care.
* Stay updated on the latest developments in physical therapy practices and medical terminology.
*Qualifications*
* Degree in Physical Therapy from an accredited institution.
* Valid OR state licensure or eligibility for licensure as a Physical Therapist.
* Knowledge of anatomy and physiology relevant to physical therapy practices.
* Experience or training in geriatrics, home community care, outpatient services, and orthopedics is preferred.
* Familiarity with medical terminology and acute care environments is advantageous.
* Strong communication skills with the ability to educate patients effectively.
* A compassionate approach with a focus on patient-centered care.

Pay: $47.00 - $55.00 per hour
