Job Summary
We are seeking a dedicated and compassionate Case Manager to join our team. In this role, you will be responsible for assessing the needs of clients, developing individualized service plans, and connecting them with the resources and support they need to thrive. You will play a key role in ensuring clients receive the necessary assistance to maintain housing stability and improve overall well-being.
Schedule: Mon- Fri 9am-5pm
Location: Brooklyn, NY
Responsibilities Include:
* Provide case management services to clients, including assessment, planning, and coordination of services via use of EBP, TIC, MI
* Assist clients to achieve housing and recovery goals
* Conduct psychiatric evaluations
* Perform other related duties, as needed.
Job Requirements:
* Bachelor’s degree in Social Work, Psychology, Sociology, or a related field.
* 2+ years of work experience in case management, social services, or supportive housing.
* Strong communication skills, both verbal and written.
* Ability to work independently and as part of a team.
* Knowledge of supportive housing programs and community resources is a plus.
* Strong organizational skills and attention to detail.
* Compassionate, patient, and professional demeanor when working with clients.
