*Position:* Registered Professional Physical Therapist
*Compensation:* 100k/year with opportunity for weekly bonus plus mileage reimbursement $0.65 per mile
*Type:* Full-Time
*Role Summary:* We are seeking a skilled and compassionate Physical Therapist to join our team on a Full-Time basis. Your primary responsibility will be to provide coordinated, high-quality care to patients of all age groups, aiming to restore or maintain their well-being.
*Key Responsibilities:*
* Develop, implement, and evaluate individualized treatment plans based on medical referrals and thorough patient evaluations.
* Conduct physical therapy programs to improve patient mobility and reduce pain.
* Collaborate with a multidisciplinary team to ensure comprehensive patient care.
* Document and complete clinical notes, phone orders, and other necessary records promptly.
* Revise patient plans of care (POC) based on continuous assessment and feedback.
* Supervise physical therapy assistants as required.
* Apply infection control principles and standard precautions during patient care.
* Ensure effective communication with physicians and other healthcare professionals involved in patient care.
* Educate patients and families on therapy techniques, use of equipment, and safety protocols.
*Qualifications:*
* *Education:* Bachelor of Science in Physical Therapy from an APTA-approved program.
* *Licensure:* Active state license as a Physical Therapist in Texas.
* *Experience:* 1-2+ years in Home Health and Manual Therapy required.
* *Skills:* Excellent therapy skills, interpersonal abilities, current CPR certification, and knowledge of durable medical equipment.
* *Transportation:* Reliable transportation with valid auto liability insurance and a current driver’s license.
*Essential Functions:*
* Perform ongoing clinical assessments to determine patient needs and functional levels.
* Develop and evaluate care plans in collaboration with patients, their representatives, and caregivers.
* Document and complete clinical records timely and accurately.
* Provide physician-ordered services and oversee patient care as needed.
* Use effective interpersonal skills to enhance patient care and collaboration.
* Promote the agency's philosophy and policies through dedicated service and professional growth.
