*What You’ll Do*
* Deliver *hands-on patient care* in a skilled nursing facility
* Administer medications and manage pain per physician orders
* Monitor residents’ conditions and report changes promptly
* Complete accurate documentation for admissions, transfers, and discharges
* Communicate effectively with families and healthcare teams
* Ensure compliance with *infection control and safety standards*
*What We’re Looking For*
* Graduate of an accredited *LPN program*
* Active *New York State LPN license*
* Current *CPR certification*
* Ability to thrive in a *fast-paced, team-oriented environment*
* Strong communication and attention to detail
